Right heart catheterization (RHC) is required for confirmation of the diagnosis of pulmonary hypertension (PH) and to asses the prognosis and etiology. Pulmonary vasoreactivity testing (PVT) is one of the most crucial parts of RHC especially in the suitable patient subsets of pulmonary arterial hypertension (PAH) defined by the most recent guidelines. PVT has substantial importance in defining the most appropriate treatment modality. The aim of this review is to provide a comprehensive review of the literature about PVT and the agents used in PVT, to emphasize the importance of this procedure when done with correct indications with a correct technique and to contribute in the accurate management of PH patients.

Purpose: Obstructive sleep apnea-hypopnea syndrome (OSAHS) is the most common sleep-related breathing disorder. Longer term, repeated episodes of hypercapnia and hypoxemia during sleep are associated with inflammatory and atherosclerosis-related factors. The aim of this study was to explore the effect of continuous positive airway pressure (CPAP) therapy on cerebral vasoreactivity and early atherosclerosis in patients with severe OSAHS.

Introduction: Despite shared features with pulmonary arterial hypertension, acute vasoreactivity in pulmonary hypertension with interstitial lung disease (PH-ILD) is not well characterised, including its potential ability to predict therapeutic outcomes. We sought to determine whether acute vasoreactivity in PH-ILD to oxygen (O) and inhaled nitric oxide (iNO) predicts inhaled treprostinil (iTre) outcomes.
Materials And Methods: In this retrospective cohort analysis, we identified treatment-naive PH-ILD patients with vasoreactivity testing using O and O+iNO.

Intracoronary acetylcholine (ACH) testing is clinically useful to diagnose the presence of the coronary vasomotor disorders coronary endothelial dysfunction and coronary epicardial/microvascular spasm. In Western countries, continuous intracoronary injection of ACH for 2-3 minutes without a pacemaker is the usual method, while rapid injection of ACH for 20-30 seconds with a pacemaker is the traditional procedure in Japan. Coronary microvascular spasm is often observed in Western populations, whereas coronary epicardial spasm is frequently seen in Japanese subjects.
