Objective: To investigate the effects of nonsteroidal antiinflammatory drug (NSAID) therapy and water immersion on hypothalamic-pituitary-adrenal (HPA) axis function in rheumatoid arthritis (RA).

Methods: Plasma levels of adrenocorticotropic hormone (ACTH) and serum and urine levels of cortisol were compared in untreated RA patients, NSAID-treated RA patients, and healthy control subjects.

Results: ACTH levels were significantly higher in untreated RA patients (mean +/- SEM integrated area 11,377 +/- 5,246 hours ng/liter) than in NSAID-treated RA patients (2,285 +/- 388 hours ng/liter) or healthy controls (1,845 +/- 35.

To evaluate the effect of topical hydrocortisone therapy on cortisol secretion, the plasma cortisol response to a 2 h adrenocorticotropic hormone (ACTH) test was determined in 17 children with dermatitis. Percutaneous absorption of hydrocortisone was measured in the acute phase of dermatitis by a 4 h absorption test. Two-hour plasma cortisol in the ACTH test correlated inversely with the increment of plasma cortisol in the absorption test.

Recent studies are reporting on an influence of ACTH and prolactin in physiological dosages on the intensity of the immune response in the animal model of adjuvant arthritis. Therefore, we studied the circadian secretion patterns of ACTH, cortisol and prolactin by measurements throughout a 24-hour cycle in two-hour intervals in patients with rheumatoid arthritis with different inflammatory activity of the disease. We only investigated patients who never before were treated with corticosteroids and drugs like gold, d-penicillamine or immunosuppressive drugs.
