Rupture of huge thoracic aortic aneurysm in a young man: Case report and literature review.

J Forensic Leg Med

January 2025

Department of Diagnostics and Public Health, Section of Forensic Medicine, University of Verona, P.le L.A. Scuro 10, 37134, Verona, Italy.

Thoracic aortic aneurysms are considered more dangerous than abdominal aneurysms because they are often silent until rupture and, therefore, are more challenging to diagnose and have a high mortality rate. In addition, a thoracic aneurysm differs from an abdominal one in terms of causes and risk factors: the former is associated with the degeneration of the vessel's middle tunica, while the latter is related to atherosclerosis. We report the case of the sudden death of a 20-year-old man, with no apparent risk factors and suffering only from a persistent cough for a month, in whom the autopsy revealed the rupture of a massive aneurysm of the ascending thoracic aorta.

Felty's syndrome (FS) is a rare and complex condition most commonly seen as a complication of longstanding rheumatoid arthritis (RA), characterized by a triad of RA, splenomegaly, and neutropenia. Diagnosing FS can be challenging due to its diverse clinical presentations and overlap with other hematologic and autoimmune conditions. We report a 47-year-old male with a history of severe anemia, recurrent blood transfusions, and a chronic leg ulcer.

Waterhouse-Friderichsen Syndrome (WFS) is a rare but life-threatening condition characterized by massive adrenal hemorrhage. WFS represents one of the features of the Overwhelming Post-Splenectomy Infection, which occurs any time after spleen removal and is recognized as the most serious complication in asplenic patients. We report a fatal case of WFS resulting from Streptococcus pneumoniae infection in a vaccinated and splenectomized patient.

Purpose: We evaluated (1) the combined effects of cold stimulation and voluntary breath holding (apnea) on heart rate, blood pressure, blood flow and vascular responses in dynamically exercising muscles in humans, and (2) if some interactions exist between cold stimulation and apnea on the cardiovascular responses.

Methods: Nine males and 1 female performed three trials entailing a dynamic two-legged knee extension exercise at a constant workload that elicited heart rates around 100 beats min. During the trials the participants performed either: (1) immersed their right hand into ice water maintained at 4 °C (cold pressor test; CPT); (2) performed maximal-duration apnea; and (3) performed a combination of CPT and apnea.

This article presents two cases of fatal injuries from rifled weapons with unconventional shapes of inlets and exits mismatched with the distance of firing. According to forensic literature, in long-distance ranges, we expected to see rounded entry wounds smaller than bullet size, circular in shape without associates with limited damage in the tissues. In the first case, there were large wounds which did not match a distance of more than 2 m, while in the second case, the distance was more than 30 m with large wounds 21 × 10 cm and massive damage to bones and lungs.
