The peril of incorrect diagnostic labelling is highlighted by this case of acute respiratory distress caused by a retrosternal recurrent goitre. An initial clinical diagnosis which cannot be fully validated on investigation with unexpected or poor response to treatment should prompt consideration and investigation for an alternative explanation.

Stress and psychological disorders are substantial public health concerns, necessitating innovative therapeutic strategies. This study investigated the psychophysiological benefits of nature-based soundscapes, drawing on the biophilia hypothesis. Using a randomized, acute cross-over design, 53 healthy participants experienced either a nature-based or a reference soundscape for 10 min, with a 2-min washout period.

We assessed the agreement among respiratory physicians on the presence and changes over time of audible crackles collected in a prospective longitudinal cohort of patients with fibrotic ILD.
Methods: Lung sounds were digitally recorded at baseline and after 12 months at eight anatomical sites.
