Aims: Sometimes, mastitis needs to be differentiated from carcinoma because of its association with induration and with ultrasound findings (such as low-echo lesions) that resemble those in carcinoma. The aim was to define this type of mastitis and to examine 18 cases to clarify its clinicopathological features.

Methods And Results: All cases were categorized into three types: non-specific mastitis with neutrophilic infiltration (n = 7); non-specific mastitis with lymphoplasmacytic infiltration (n = 9); and granulomatous lobular mastitis (n = 2). The three types of mastitis presented similar ultrasound findings and shared certain histological features including fibrosis and diffuse or lobulocentric inflammation. Granulomatous lobular mastitis showed specific clinicopathological features including lobulocentric inflammation with giant cells, diffuse IgG4+ plasma cells, and also a high level of serum IgG4.

Conclusions: Granulomatous lobular mastitis could be categorized into IgG4-related and non-IgG4-related granulomatous lobular mastitis. IgG4 immunohistochemistry serum IgG4 might be useful for diagnosis of IgG4-related granulomatous lobular mastitis and could help to avoid overtreatment such as wide excision.

Cystic neutrophilic granulomatous mastitis (CNGM) is a rare type of granulomatous lobular mastitis (GLM) with a distinct histologic pattern characterized on histopathology by clear lipid vacuoles lined by peripheral neutrophils ("suppurative lipogranulomas"), often containing gram-positive bacilli and strongly associated with Corynebacterial infection (in particular, Corynebacterium kroppenstedtii). Cystic neutrophilic granulomatous mastitis has a distinct histopathologic appearance, but the imaging appearance is less well described and has been limited to case reports and small case series published primarily in pathology literature. Mammographic findings of CNGM include focal asymmetry, skin thickening, and irregular or oval masses.

Article Synopsis
  • Idiopathic Granulomatous Mastitis (IGM) is a rare benign inflammatory condition affecting the breast, often challenging to diagnose.
  • A case study highlights a 45-year-old woman who developed IGM after neoadjuvant chemotherapy for stage II invasive ductal carcinoma, eventually requiring modified radical mastectomy and steroid treatment.
  • There is limited literature on IGM, and existing treatment guidelines are not well-established, complicating management approaches.

Article Synopsis
  • The study investigated the effects of traditional Chinese medicine (TCM) on granulomatous lobular mastitis (GLM) using MRI to assess changes in clinical manifestations before and after treatment.
  • Results showed significant reductions in symptoms like lumps, pain, and menstrual irregularities among the 29 women studied, confirming the effectiveness of TCM.
  • MRI findings revealed notable changes in lesion characteristics and other clinical features, indicating that TCM therapy can provide reliable improvements for patients with GLM.
