This clinical lesson describes three patients with problems after a total laryngectomy. Patient A suffered from acute tracheitis and narrowing of the tracheostoma. Patient B experienced acute dyspnea due to thick mucus plugs.

Introduction And Objective: For patients requiring prolonged mechanical ventilation, tracheostomy becomes necessary, which may be performed through surgical or percutaneous methods. In this study, we used three different methods of percutaneous dilatational tracheostomy. Our goal was to identify anthropometric parameters relevant for the correct position of the tracheostomy tube.

Background: The rigid tracheotomy endoscope (TED) was recently introduced to improve the fiberoptic technique during percutaneous dilatational tracheotomy (PDT) in critically ill patients. The aim was to evaluate the long-term complications of PDT using TED equipment in a prospective multicenter investigation.
Methods: One hundred eighty adult patients underwent PDT using TED in four German hospitals.

Background: Advances in modern intensive care have led to a sharp increase in the number of tracheotomies performed in intensive care units. In contrast to surgical epithelial tracheostomy (ST), a less sturdy stoma is created by means of percutaneous dilation of tracheotomy (PDT). The advantages of a PDT compared to ST are the simpler logistical efforts, associated cost reduction and minimally invasive nature of the process.

Background: An iatrogenic tracheoesophageal (TE) fistula is one possible complication after total laryngectomy with flap reconstruction. We used sternocleidomastoid (SCM) rotation flap to close a TE fistula.
Methods And Results: A 69-year-old man with laryngeal cancer underwent total laryngectomy with radial forearm free flap reconstruction.
