Purpose: This is a prospective study reporting on modified deep sclerectomy (DS) using the Ex-PRESS LR-50 in combined cataract and glaucoma surgery.

Methods: Twenty-four eyes of 24 patients with medically uncontrolled glaucoma underwent modified DS. After phacoemulsification and intraocular lens implantation, the Ex-PRESS LR-50 was inserted into the anterior chamber under a scleral flap. A partial posterior DS was performed to provide an intrascleral bleb. The complete success rate was intraocular pressure (IOP) without medication =18 mmHg; the qualified success rate was IOP =18 mmHg with or without medication. Postoperative bleb management (subconjunctival mitomycin C injections ± needling) was performed in case of postoperative hypertension or bleb fibrosis.

Results: Follow-up was 40.1±10.8 (mean ± SD) months. Preoperatively, IOP was 18.1±5.3 mmHg, best-corrected visual acuity (BCVA) was 0.6±0.3, and number of medications was 2.3±1.1. The IOP decreased by 25.4% at 24 months and by 27.0% at 48 months. At 24 months, 19 patients (86.3%) achieved a BCVA of 0.5 or better, and at 48 months the mean BCVA was 0.7±0.3. At the last visit, the mean number of medications was 0.6±0.8 (p<0.05). The complete and qualified success rates were 45.6% and 85.2%. Mitomycin C injections were performed in 5 eyes. No conjunctival erosions over the Ex-PRESS LR-50 were seen.

Conclusions: The Ex-PRESS LR-50 inserted into the anterior chamber after modified DS efficiently lowers IOP in combined surgery, preventing conjunctival erosion, a significant complication when using this device without scleral flap coverage.

Background: Patient safety is important in daily anesthesia practices, and providing deep anesthesia is difficult. Current debates on the optimal anesthetic agents highlight the need for safer alternatives. This study was justified by the need for safer and more effective anesthetic protocols for outpatient hysteroscopic procedures, particularly those conducted outside the operating room.

Modifiable risk factors for culture positivity after primary Latarjet procedure. Should we change any practice?

J Shoulder Elbow Surg

Assistance Publique Hôpitaux de Paris (APHP) Ambroise Paré, Paris, 75005, France.

Background: The prevalence of positive cultures in primarily operated shoulders represents a growing concern. Previous studies have documented high rates of culture positivity in primary prosthetic shoulder surgery, yet the Latarjet procedure has not been specifically explored in this context. The aim of this study was to identify the rate of culture positivity in patients treated with a primary Latarjet procedure, and to identify risk factors for culture positivity.

The secondary near-infrared region (NIR-II) fluorescence imaging-guided photothermal therapy (PTT) offers a noninvasive and light-controllable treatment option for deep-seated cancers. However, the development of NIR-II photothermal agents (NIR-II PTAs) that possess the desired properties of high molar absorption coefficient (ε), fluorescence quantum yield (QY), and photothermal conversion efficiency (PCE) remain a challenge due to the contradiction between radiative and nonradiative processes. Herein, we propose a novel side-chain heteroatom substitution engineering strategy to simultaneously enhance ε, QY, and PCE by modifying the molecular planarity.

Introduction: Adults with relapsed or refractory Philadelphia chromosome-positive B-cell precursor acute lymphoblastic leukaemia (R/R Ph+ BCP-ALL) have a dismal outcome. Blinatumomab as a single agent has shown activity in R/R Ph- BCP-ALL, and second or third-generation tyrosine kinase inhibitors (TKIs) can produce high remission rates in Ph+ leukaemias. We aimed to assess the activity of blinatumomab and TKI in combination with intensive chemotherapy in the relapsed or refractory setting.

The intrinsic spontaneous and piezoelectric polarizations of GaN lead to the formation of triangular wells and barriers, resulting in the manifestation of chaotic transport models in GaN quantum well intersubband transition (ISBT) infrared detectors and giving rise to various adverse effects. The APSYS software was utilized to construct a novel GaN quantum well ISBT infrared detector in this study. By endeavoring to modify the quantum well structure, our objective was to precisely adjust the energy level of the first excited state (E1) to align with the apex of the triangular barrier.
