Analysis of the postoperative results of carotid endarterectomy under local anesthesia in 40 patients showed the possibilities of this type of anesthesia in evaluation of cerebral collateral circulation. The method allowed the use of temporary intraluminal shunt to be rejected in all patients and intraoperative ischemic complications on the part of the brain were not encountered in any of them. A high correlation between the results of EEG and the intraoperative evaluation of tolerance of the brain to ischemia was registered.

Learnability of Ultrasound-Guided Locoregional Anesthesia for Carotid Endarterectomy.

J Clin Med

Department of Anesthesiology, Heidelberg University Hospital, Medical Faculty, University of Heidelberg, 69120 Heidelberg, Germany.

: There is an ongoing debate about the most advantageous anesthesia technique for carotid endarterectomy (CEA). From an anesthesiologic perspective, locoregional anesthesia (LRA) appears to offer significant benefits. However, the learning curve and complication rates for anesthesiologists newly performing ultrasound-guided LRA for CEA remain unclear and are to be examined in greater detail in this study.

Background: The Society for Vascular Surgery guidelines recommend carotid revascularization within 14 days of symptom onset for neurologically stable stroke patients. However, in the United States, insurance status may affect surgical timing, although large-scale studies are lacking. Using the Vascular Quality Initiative database, we evaluated the impact of insurance status on surgical wait times for patients with symptomatic carotid disease.

Article Synopsis
  • The research investigates the link between preoperative severe asymptomatic carotid artery stenosis (ACAS) and the occurrence of severe multi-organ dysfunction in patients who underwent off-pump coronary artery bypass grafting (OPCAB).
  • In a study involving 562 patients, 63 (11.2%) were found to have severe ACAS, which was associated with higher rates of organ dysfunction and increased 30-day adverse outcomes post-surgery.
  • The findings suggest that severe ACAS is an independent risk factor for severe multi-organ dysfunction and major cardiovascular events, including strokes, following OPCAB procedures.

Article Synopsis
  • Anastomotic pseudoaneurysms of interposition vein grafts are rare but serious complications in vascular surgery, particularly concerning the carotid arteries, requiring urgent intervention to avoid critical issues like rupture and thromboembolism.
  • A case report details a 49-year-old male who developed a false aneurysm after a carotid surgery complication and successfully underwent open surgical repair involving the removal of the affected graft and reconstruction using a vein graft from the subclavian artery.
  • The conclusion emphasizes that while endovascular techniques exist, open surgical repair remains the standard treatment for these pseudoaneurysms due to its ability to effectively address infection and ensure better long-term outcomes, suggesting that further research should continue to refine surgical strategies.
