Dens invaginatus is a developmental anomaly. The nonsurgical endodontic treatment of a type 3 dens invaginatus with a large periradicular lesion in a maxillary left lateral incisor of a 10-year-old girl is presented. In spite of difficult diagnosis and treatment of type 3 dens invaginatus, nonsurgical treatment was performed successfully. Resolution of the periradicular lesion and apical closure was observed after 1-year follow-up.

Aim: This report highlights successful management of a rare case of a mandibular premolar with Oehler's Type IIIb Dens Invaginatus (DI) and peri-invagination periodontitis with guided intentional replantation (IR) without root canal treatment.
Methods: A 22-year-old female patient reported with discomfort and a sinus tract associated with tooth #21. Clinical examination and cone beam computed tomography revealed tooth #21 had an Oehler's Type IIIb DI with peri-invagination periodontitis.

Background: Dens invaginatus is a developmental abnormality originating from tooth development, including coronal invaginatus and radicular invaginatus. The reported incidence varies greatly due to diagnostic techniques, classification criteria and race. The incidence of dens invaginatus in China was not clear, and the impact of dens invaginatus on periodontal support tissue were rarely reported.

Dens Invaginatus (DI) is a developmental anomaly which eventually leads to pulp necrosis and has several clinical implications in sufficient instrumentation and obturation of the root canal system. The present clinical report presents a rare case of a maxillary canine affected with DI leading to pulp necrosis combined with a previous dental trauma, which also led to irreversible pulp damage of the adjacent lateral incisor. A 14-year-old male patient with a history of dental trauma at the right maxillary region, one year earlier, was referred with pain and swelling at the apical area of the right maxillary canine.
