The authors describe their experience in using cycloferon in 27 patients with pyoseptic abdominal pathology. For comparative evaluation of the efficacy of the immunomodulator, they examined 34 patients who received conventional drug therapy. Immunomodulatory therapy was performed, by taking into account the data on the immune status and severity rated by the SIRS and SAPS. Indications for immunomodulatory therapy were defined. The results of immunomodulation suggest that the use of cycloferon in the complex therapy of patients with pyoseptic abdominal diseases makes it possible to enhance the efficiency of treatment and to reduce the number of postoperative complications.

The in-hospital infections are one of the most serious problems of medicine, especially if patients have a background immunosuppression of various genesis conditioned by both disease itself and corresponding therapy. The detection of presence of infection and identification of agent and detection of its resistance are needed for choosing adequate therapy. At that, high heterogeneity of strains and multiple resistance of nosocomial infections to antibiotics and antimicrobial pharmaceuticals and standardization of antibacterial prevention and number of other causes becomes an obstacle for both determination of medicinal sensitivity of bacterium and for identification of pathogen itself in patient.

The authors retrospectively analysed therapeutic results of treatment in a total of 749 patients of the Pyoseptic Surgery Unit with signs of lower limb critical ischaemia. Of these, 101 (13.5%) patients were found to have a pronounced septic process (successfully arrested in 88 patients).

Survival was estimated at 83% (88 patients).
