Surgical treatment of ruptured metastatic pleomorphic leiomyosarcoma.

Asian Cardiovasc Thorac Ann

Department of Cardiothoracic Surgery, St. Vincent's Hospital Melbourne, Fitzroy, Victoria 3065, Australia.

Published: June 2010

A rare case of primary cardiac leiomyosarcoma was diagnosed in a 21-year-old man who presented with a groin mass thought to be a sebaceous cyst. Histopathology revealed a high-grade pleomorphic leiomyosarcoma. Combined positron-emission and computed tomography showed a large metabolically active left atrial mass with multiple metastases. Major debulking resection was undertaken, followed by radiation and chemotherapy. At 13 months postoperatively, limited spread has been detected, and the patient had no limitation in daily life.

There has been noteworthy progress in molecular characterisation and therapeutics in soft tissue sarcomas. Novel agents have gained regulatory approval by the FDA. Examples are the tyrosine kinase inhibitors avapritinib and ripretinib in gastrointestinal stromal tumours (GIST), the immune check point inhibitor atezolizumab in alveolar soft part tissue sarcoma, the γ-secretase inhibitor nirogacestat in desmoid tumours, the NTRK inhibitors larotrectinib and entrectinib in tumours with fusions, the mTOR inhibitor nab-sirolimus in PEComa, and the EZH-2 inhibitor tazemetostat in epithelioid sarcoma.

The diagnosis of primary leiomyosarcoma (LMS) of bone is generally established based on integrative findings of morphologic features, immunohistochemical staining, and clinical and radiological findings. There are no specific genetic alterations that can be used to confirm the diagnosis of LMS in challenging diagnoses of bone sarcomas with myogenic differentiation. In this study, we assessed the utility of a DNA methylation-based classifier as an ancillary diagnostic tool for subclassifying bone sarcomas with myogenic differentiation.

The present study was a retrospective single-center study. A total of 81 patients diagnosed with metastatic soft tissue sarcoma were included who received pazopanib therapy. Clinical data, including age at diagnosis, histological subtype, treatments received before pazopanib, number of metastatic sites at the time of initiation of treatment, progression-free survival and overall survival time under pazopanib treatment, side effects and response evaluation in follow-up imaging after initiation of pazopanib therapy, were recorded.

Article Synopsis
  • Liposarcomas are the most common type of soft tissue sarcoma, primarily found in deep tissues and the retroperitoneum.
  • They include various classifications such as atypical lipomatous tumors, well-differentiated liposarcoma (WDL), dedifferentiated liposarcoma (DDL), myxoid liposarcoma, and pleomorphic liposarcoma.
  • DDL is especially common in the retroperitoneum and can have both well-differentiated lipomatous and dedifferentiated components, sometimes resembling other aggressive tumors, and can even differentiate into several other cancer types or form bone tissue.

Primary Leiomyosarcoma of the Distal Ulna: A Case Report.

J Orthop Case Rep

December 2024

Department of Orthopaedics, Subharti Medical College and Hospital, Meerut, Uttar Pradesh, India.

Article Synopsis
  • Primary leiomyosarcoma in bone is a rare tumor that mainly occurs in older adults and may be a metastasis; it requires careful evaluation and broad surgical excision as treatment.
  • A case study describes a 50-year-old man with a painful mass in his left wrist, diagnosed with primary leiomyosarcoma of the distal ulna and treated with wide resection and stabilization.
  • This type of tumor, although uncommon, should be considered when diagnosing lytic aggressive bone lesions in middle-aged patients, with surgery being the primary treatment method.
