Objectives: To describe the use of pulsed fluoroscopic guidance, to perform endoscopic procedures in pregnant women, by inverting the fluoroscope's c-arm using a lead thyroid collar to shield the fetus from the direct X-ray beam. The use of radiation during treatment of pregnant patients with urolithiasis remains a recurring dilemma.

Methods: Between May 2006 and December 2008, endoscopic treatment due to ureteral stones was attempted in 8 pregnant women. In all cases, we use an inverted fluoroscope's c-arm during endoscopic treatment associated with 2 lead neck thyroid collars to shield the uterus, protecting the fetus from direct radiation. Indication for treatment was symptomatic ureteral stones unresponsive to medical treatment in 7 and persistent fever in 1.

Results: Mean ureteral stone size was 8.1+/-4.8 mm, located in the left ureter in 5 (62.5%) cases. Three (37.5%) patients had stone located in the upper ureter, 2 (25%) in the middle ureter, and 3 (37.5) in the distal ureter. In 6 cases, ureteral stones were treated using the semi-rigid ureteroscope, whereas in 1 case a flexible ureteroscope was needed. One woman was treated with insertion of a double-J stent due to associated urinary infection. No women has early delivery related to the endoscopic procedure, and all neonates were perfectly normal.

Conclusions: We present a technique for endoscopic procedures in pregnant women inverting the fluoroscope's c-arm and protecting the fetus from the direct X-ray beam. This practical approach should be specially considered when no portable ultrasound and radiologic assistance in available in the operating room.

Sublober resection of small peripheral lung lesions using video-assisted thoracoscopic surgery may require marking and confirmation using 3D imaging in the interventional radiology suite or in the hybrid operating room (HOR) before surgery is started. We report a novel approach for intraoperative transbronchial metallic coil marking followed by thoracoscopic wedge resection in a conventional operating room under mobile 3D C-arm guidance. Under general anesthesia, an ultrathin video-bronchoscope was inserted into an objective bronchus guided with virtual bronchoscopic navigation, and a coil-feeding microcatheter was introduced through the bronchoscope's channel.

Background: The neurolytic celiac plexus block (NCPB) can be introduced through the posterior para-aortic, anterior para-aortic, posterior transaortic, or endoscopic anterior para-aortic puncture approach, as well as the posterior approach via the intervertebral disc. To reduce the complications of puncture, this block's original manual blind puncture technique can be improved upon by using a C-arm fluoroscope, computed tomography (CT), or an ultrasound, the last of which may be endoscopic.

Objective: To observe the distribution of absolute alcohol and its analgesic effect on cancer-induced upper abdominal visceral pain during percutaneous NCPB through the anterior and posterior diaphragmatic crura under CT guidance.

Impact of Leg Position on Measurements Used to Detect Femoral Component Subsidence in THA.

Orthopedic Surgery Artificial Intelligence Laboratory, Department of Orthopedic Surgery, Mayo Clinic, Rochester, MN, USA.

Background: A fully automated artificial intelligence-based tool was developed to detect and quantify femoral component subsidence between serial radiographs. However, it did not account for measurement errors due to leg position differences, such as rotation or flexion, between comparative radiographs. If there are small differences in rotation or flexion of the leg between comparative radiographs, the impact on subsidence measurement is unclear.

Article Synopsis
  • * The procedure was guided using fluoroscopy, but shortly after, she lost consciousness and became quadriplegic, leading to her transfer for treatment which included standard therapy and alternative methods like acupuncture.
  • * Remarkably, despite the initial severity of her condition, she experienced significant neurological recovery over eight months, regaining normal strength in her limbs with only minor residual symptoms.

Article Synopsis
  • C-arm fluoroscopy is crucial for accurately visualizing the irregular anatomical structures during surgeries involving the femoral neck, as traditional imaging techniques are often inadequate.
  • The study analyzed the femoral neck's anatomy by measuring specific angles related to its surfaces and tested optimal C-arm angles for better fluoroscopic imaging.
  • Results indicated the femoral neck's complex shape and determined the best angles for visualizing various surfaces, with an emphasis on the effectiveness of Kirschner wires in imaging during procedures.
