Importance: The Centers for Medicare & Medicaid Services (CMS) Bundled Payments for Care Improvement (BPCI) program was launched in 2013 with a goal to improve care quality while lowering costs to Medicare.

Objective: To compare changes in the quality and outcomes of care for patients hospitalized with heart failure according to hospital participation in the BPCI program.

Design, Setting, And Participants: This cross-sectional study used a difference-in-difference approach to evaluate the BPCI program in 18 BPCI hospitals vs 211 same-state non-BPCI hospitals for various process-of-care measures and outcomes using American Heart Association Get With The Guidelines-Heart Failure registry and CMS Medicare claims data from November 1, 2008, to August 31, 2018.

Importance: In patients with severe aortic valve stenosis at intermediate surgical risk, transcatheter aortic valve replacement (TAVR) with a self-expanding supra-annular valve was noninferior to surgery for all-cause mortality or disabling stroke at 2 years. Comparisons of longer-term clinical and hemodynamic outcomes in these patients are limited.

Objective: To report prespecified secondary 5-year outcomes from the Symptomatic Aortic Stenosis in Intermediate Risk Subjects Who Need Aortic Valve Replacement (SURTAVI) randomized clinical trial.

The Cardiac Pacemaker Story-Fundamental Role of the Na/Ca Exchanger in Spontaneous Automaticity.

Front Pharmacol

April 2020

MTA-SZTE Research Group of Cardiovascular Pharmacology, Hungarian Academy of Sciences, Szeged, Hungary.

Article Synopsis
  • Recent studies show that the Na/Ca exchanger (NCX) is crucial for the automaticity of the sinus node, challenging the old view that it was only regulated by the "funny current."
  • Evidence from experiments and modeling supports the importance of NCX in sinus node pacemaking, leading to a consensus on the coupled-clock mechanism.
  • The review summarizes the history and advancements regarding NCX’s role, its influence on calcium homeostasis, and highlights the challenges faced in studying its function due to complex interactions in calcium handling.

Opinions on the prevention of infective endocarditis (IE) have significantly changed in recent years. A gradual departure from antibiotic prophylaxis can be observed, both in terms of the spectrum of procedures and the profile of individuals in whom it is indicated.
