Introduction: In this case report, we would like to discuss about a 51-year-old gentleman, who had underwent patella tension band wiring for transverse patella fracture 3 years ago and presented with a painful knee with broken cerclage wire with a fragment migrating into the joint.

Case Report: The aim of the study was to highlight the importance of higher imaging (Computed tomography) in deciding whether the retrieval of the fragment can be attempted through a minimally invasive arthroscopic technique.

Result: CT of the knee joint with 3D reconstruction was done and it showed the broken migrated fragment to be intra-articular and intracapsular.

Article Synopsis
  • The article discusses a new strategy for improving hemostasis during intracapsular enucleation (ICE) surgeries for cervical sympathetic chain schwannoma (CSCS), addressing previous challenges like diffuse hemorrhage.* -
  • A review of 8 CSCS cases revealed that the new preemptive approach for managing blood vessels significantly reduced both operation time and recovery period for postoperative Horner's syndrome (HS).* -
  • Results showed that while HS occurred in most patients, recovery was notably quicker with the new strategy, and no recurrences of CSCS were observed during the 20-month follow-up.*

Cerclage wiring and tension band wiring are commonly utilized in orthopedic surgeries for patellar fractures, but wire breakage is a recognized complication. This report presents a rare case where a broken cerclage wire exhibited intraarticular intracapsular migration, prompting open removal adjacent to the medial femoral condyle after unsuccessful attempts at arthroscopic extraction. A 50-year-old male with a history of patellar fracture fixation using cerclage and tension band wiring, presented with persistent knee pain and restricted motion.

Purpose: To study whether it is better to perform or not a myomectomy, in terms of surgical and reproductive outcomes in patients of advanced reproductive age, by an observational prospective study in university-affiliated and Community Hospitals.

Materials And Methods: 40 years and older patients affected by non-submucous symptomatic uterine fibroids and desiring future fertility were enrolled and treated by laparoscopic intracapsular myomectomy by (LIM) or by open laparotomy (OIM), or by a non-surgical management as control group, while attempting to conceive. The primary outcome measures were fibroid characteristics, pre- and post-surgical parameters, pregnancy achievement; the secondary outcome measures were the spontaneous or ART pregnancy outcomes, eventual week of abortion and type of delivery.
