[Brisement forcé after knee replacement].

An important goal for rehabilitation after knee replacement is achievement of good capacity of flexion so that the patient can cope with his everyday needs. If this fails, brisement forcé may be undertaken; the knee joint being moved thoroughly with the patient in general anaesthesia. The results of brisement forcé in 32 knees showed that brisement forcé had an immediate good effect on the patients' ability to flex the joints but that the benefit was short-lived and possible positive effect on the final mobility appears to be less certain. Complaints of pain prior to manipulation and audible rupture of adhesions and tenacious resistance during manipulation were deleterious prognostic signs for good mobility in the final result.

Article Synopsis
  • Trochlear dysplasia increases the risk of recurrent patellar instability and complications like osteoarthritis, which can be addressed through a specific treatment approach involving trochleoplasty and medial patellofemoral ligament reconstruction (MPFL-R).
  • This study evaluates patient outcomes over 5 years after surgery, analyzing metrics like patellar stability and patient-reported quality of life.
  • Among 131 patients, significant improvements were observed in knee function metrics, although 2% experienced dislocations post-surgery, and 38% required additional surgeries.

Purpose: The purpose of this study was to create a treatment protocol for cases of heterotopic ossification (HO) of the temporomandibular joint (TMJ), particularly those refractory to current TMJ HO protocols. In addition, we demonstrate the success of this protocol on a unique case of recurrent HO that failed multiple TMJ HO protocols in the setting of an improvised explosive device (IED) blast in a wounded warrior.

Methods: An electronic literature review was conducted via PubMed and Web of Science.

Hemidiaphragmatic Paresis Following Interscalene Brachial Plexus Block With 2-Point Injection Technique.

Department of Anesthesiology and Pain Medicine, Kangdong Sacred Heart Hospital, Hallym University College of Medicine, Seoul, Republic of Korea.

Background: An interscalene brachial plexus block is a commonly conducted nerve block for anesthesia and analgesia in shoulder surgery. Due to its proximity to the targeted nerve, the phrenic nerve, which innervates the diaphragm, is typically inadvertently blocked by ventral spread of the local anesthetic. Although hemidiaphragmatic paresis is tolerable in healthy patients, it would be an irreversible risk to patients with compromised lung reserve.

Background: The brisement manipulation is an effective treatment for refractory shoulder stiffness. Rotator cuff tears can sometimes exist in combination with adhesive capsulitis. Arthroscopic capsular release combined with rotator cuff repair has achieved good outcomes in published reports.
