Dorsal adipofascial flaps have been used in the surgical reconstruction of complex injuries distal to the eponychial fold. Such injuries produce nail matrix devascularization/necrosis so that nail bed reconstruction can be a challenging technical problem. Irregular scarring of the nail bed and regrowth anomalies of the nail lamina can result, with both functional and cosmetic impairment of the finger. This study aimed to define the precise vascular anatomy of the dorsal adipofascial flap that has previously been used to reconstruct such complex soft tissue defects. Specifically, the purpose was to identify the key points of safe dissection for these flaps. Anatomical dissections were performed on 32 long fingers. The vascular tree was injected with suitable contrast and the distal dorsal region of the long fingers was studied.

This article presents particular occupational burn injuries resulting in a complex defect of the digits. Nine patients with exposed bone or tendon wounds on the digits were successfully treated using the pedicle adipofascial turnover flap overlaid with skin grafts. Electrical and high-temperature contact burns resulted in five and four severe cases, respectively.

Aim: Dorsal metatarsal artery perforator flap (DMTAPF) reconstructions were reviewed to assess indications for application, variation of surgical technique and the long-term impact of growth.
Method: A cohort of 15 children presented with 17 DMTAPFs on 16 feet. Patient demographics, the aetiology and timing of primary injury and details of flap reconstruction were recorded.

The superficial radial nerve (SRN) is vulnerable to injury following trauma with a high incidence of resultant nerve tether and neuroma formation. The SRN has an anatomical predisposition to neuroma formation, with research indicating that its propensity to neuroma development is out of proportion with its likelihood for injury. In addition, SRN neuromas have been described as one of the more painful and difficult neuromas to manage.
