Pseudoaneurysm of the left ventricle (LV) is a rare cardiac disease that occurs after myocardial infarction or cardiac surgery. Because patients frequently present with nonspecific symptoms, a high index of suspicion is needed to make the diagnosis. This report describes an unusual case demonstrating a large LV pseudoaneurysm after mitral valve replacement performed 30 years earlier.

A 72-year-old man with a history of aortic valve replacement (AVR), mitral valve (MV) repair, and recent valve-in-valve transcatheter aortic valve replacement (TAVR) presented with bacteremia. Computed tomography (CT) and fluorodeoxyglucose positron emission tomography (F-FDG PET) imaging noted a pseudoaneurysm, and he successfully underwent TAVR explant with aortic root and valve replacement.

Background: Redo mitral valve surgery by re-sternotomy approach has challenges such as bleeding and organ injury due to adhesion dissection, sternal bone infection, and poor field of view of mitral valve. On the other hand, redo mitral valve surgery via a right mini-thoracotomy approach appears to address these challenges. We successfully performed a double right mini-thoracotomies approach for mitral valve replacement and pseudoaneurysm repair under the beating-heart condition.
