Suppurative hidradenitis is a chronic inflammatory process that involves apocrine glands of the axial, inguinal and perianal region. We present a male patient of 47 years old who was admitted to our hospital with a complex perianal fistula with not a good evolution after surgery. Then, a large resection of the gluteus was performed, because of a big lesion extended to the perianal region. A colostomy was associated. The pathology showed an epidermoid carcinoma. This is a rare complication of the suppurative hidradenitis. The final treatment is a large resection of the lesion. The few number of cases in the literature moved us to publish-hour case.
