Coral nephrolithiasis (CN) patients treated surgically in Research Institute of Urology and hospital N 47 from 2005 to 2008 were divided into 5 groups by treatment: PCNL, PCNL+extracorporeal lithotripsy (EL), EL, open interventions, EL of the residual concrements after previous open operations. Most complications were observed after EL and they were caused by evacuation of many fragments. PCNL caused in some cases massive hemorrhage stopped by conservative methods. Open operations can effectively remove concrements in one stage but sometimes entail such severe complications as damage to adjacent organs, pleura or abdomen, hemorrhage, exacerbation or progression of chronic pyelonephritis. Results of combined surgical interventions (PCNL+EL) and EL of residual concrements after open surgery are comparable with those of EL as monotherapy if the schedule of repeat sessions is observed and saving regimens of fragmentation are used.

GeoBioMed - a new transdisciplinary approach that integrates the fields of geology, biology and medicine - reveals that kidney stones composed of calcium-rich minerals precipitate from a continuum of repeated events of crystallization, dissolution and recrystallization that result from the same fundamental natural processes that have governed billions of years of biomineralization on Earth. This contextual change in our understanding of renal stone formation opens fundamentally new avenues of human kidney stone investigation that include analyses of crystalline structure and stratigraphy, diagenetic phase transitions, and paragenetic sequences across broad length scales from hundreds of nanometres to centimetres (five Powers of 10). This paradigm shift has also enabled the development of a new kidney stone classification scheme according to thermodynamic energetics and crystalline architecture.

Aim: To determine the place of laparoscopic pyelolithotomy in surgical management of nephrolithiasis.
Materials And Methods: We analyzed 16 laparoscopic pyelolithotomies performed in our clinic over the last few years for large and stag-horn stones of anomalous kidneys (n=8), the "giant" (>30-40 cc) monolithic stones of the large extrarenal pelvis (n=3), multiple stones of PCS with the stone burden of more or equal 40 cm (n=2) and large recurrent (after PCNL) kidney stones in patients at risk of bacteriotoxic complications of percutaneous nephroscopy (n=3). The age of the patients ranged from 39 to 72 years (mean age 51+/-8 years).
This article presents the experience of applying 3D-computer simulation for treatment planning in patients with coral kidney stones. The impact of the kidney internal structure and the nephrolithiasis stage on the treatment results of the patients with staghorn stones are analyzed on the basis of authors' own experience. They note the usefulness of computer modeling in preoperative planning for improving the treatment results and preventing intraoperative complications.

Introduction into clinical practice of modern minimally invasive treatment methods changed the paradigm of treatment of urolithiasis, especially coral type nephrolithiasis - cause of 15-50% of all renal calculi.

To assess the renal function before surgery and before discharge (6 to 7 days after surgery), glomerular filtration rate (GFR) by Cockcroft-Gault equation was defined. Preoperative renal functional reserve was evaluated by method using a single oral loading of protein drugs.
