Background: The neurovascular conflict in trigeminal neuralgia is an intractable condition; medical treatment is usually of long duration and can be annoying for both patients and clinicians.

Aim: This prospective study was designed to assess the outcome of microvascular decompression (MVD) in patients with more than 3 years' history of intractable idiopathic trigeminal neuralgia (TN) and poor response to drugs.

Materials And Methods: Twenty-one patients (8 females and 13 males) with intractable idiopathic TN (group 1) underwent MVD and were followed up for 2 years. Group 2 (n = 15), which included 6 females and 9 males, received pharmacotherapy. The outcome responses of pain relief were evaluated using a 10-cm visual analog scale (VAS) and the Barrow Neurological Institute (BNI) scoring system. The patients' morbidity was recorded as well.

Results: All patients fulfilling the inclusion criteria were offered MVD surgery. Freedom from pain was achieved immediately after surgery in 95.2% (n = 20) of patients in group 1, and 90.5% (n = 19) had sustained relief over the follow-up period. There were no statistical significance recurrences or surgical complications in group 1 (P>0.5), while 53.3% (n = 8) of the subjects in group 2 showed poor response with pharmacotherapy over the same period of time and many patients experienced drug intolerance that had statistical significance (P<0.01).

Conclusion: Early MVD in TN can help patients avoid the side effects of drugs and the adverse psychological effects of long-term pharmacotherapy and prolonged morbidity.

Generalized epilepsy is classically thought of as a disease of the young and adolescent, with rarely reported cases among older adults. We aimed to analyze management and outcomes in a population sparsely described in the literature through a retrospective single-center cohort design. After excluding individuals without follow-up, we identified 151 people ≥50 years at the time of electrographically confirmed generalized epilepsy.

Background: The composite physiologic index (CPI) was developed to estimate the extent of interstitial lung disease (ILD) in idiopathic pulmonary fibrosis (IPF) patients based on pulmonary function tests (PFTs). The CALIPER-revised version of the CPI (CALIPER-CPI) was also developed to estimate the volume fraction of ILD measured by CALIPER, an automated quantitative CT postprocessing software. Recently, artificial intelligence-based quantitative CT image analysis software (AIQCT), which can be used to quantify the bronchial volume separately from the ILD volume, was developed and validated in IPF.

Rome Foundation and international neurogastroenterology and motility societies' consensus on idiopathic gastroparesis.

Lancet Gastroenterol Hepatol

January 2025

Translational Research Center for Gastrointestinal Disorders, Leuven University, Leuven, Belgium; Division of Gastroenterology and Hepatology, University Hospitals Leuven, Leuven, Belgium; Department of Molecular and Clinical Medicine, Institute of Medicine, Sahlgrenska Academy, University of Gothenburg, Gothenburg, Sweden. Electronic address:

To establish a consensus on the definition and management of idiopathic gastroparesis, international experts (selected by neurogastroenterology and motility societies and initiated by the Rome Foundation) devised 144 statements using the Delphi method, with at least 80% agreement required. This consensus defined idiopathic gastroparesis as the presence of symptoms associated with delayed gastric emptying in the absence of mechanical obstruction. Nausea and vomiting were identified as cardinal symptoms.

Article Synopsis
  • - The study investigated the clinical features and outcomes of younger patients with idiopathic pulmonary fibrosis (IPF), focusing on those under 50 years old compared to older age groups.
  • - Data was analyzed from the National Korean Health Insurance database, revealing that younger IPF patients had fewer comorbidities and were more likely to receive systemic steroids instead of antifibrotic treatments like pirfenidone.
  • - Despite younger patients having a lower mortality risk and no significant increase in acute exacerbation risk, there's a need for better-targeted treatments, including more frequent use of antifibrotics, to improve their health outcomes.

Article Synopsis
  • Rapidly progressive interstitial lung disease (RP-ILD) is a serious complication in patients with idiopathic inflammatory myopathy (IIM), often leading to poor outcomes, making the need for effective treatments essential.
  • Plasma exchange (PE) may help by quickly removing harmful substances from the blood, and this study assesses its effectiveness when combined with immunosuppressants in improving survival rates and reducing inflammatory markers.
  • Results showed an 80% survival rate at 6 months for patients undergoing PE, which was significantly better than for those not receiving PE, suggesting its potential as a beneficial treatment option for IIM-RP-ILD.
