Purulent pericarditis usually presents as an acute illness with diffuse involvement of the whole pericardium, but can rarely present as a localized effusion. Here, we present a patient with a pericardial abscess caused by Staphylococcus aureus. After surgical drainage of the abscess, the patient made a good recovery. To the best of our knowledge, this is the first case of a pericardial abscess presenting as a localized bulge of the heart contour on the chest X-ray.

A 71-year-old gentleman with prior bioprosthetic aortic valve replacement was admitted with aortic valve dehiscence and an aortic root abscess. He underwent reoperative sternotomy, aortic root, mitral valve, and hemiarch replacement. To augment hemostasis, we implanted the "Martin Mattress"-a pericardial patch sutured to the fibrous ridge within the innominate vein, superior vena cava, right atrium, right ventricular outflow tract, and pulmonary artery-which is preferred to modified Cabrol fistula techniques in infectious root pathology.

Surgery for Bentall's endocarditis poses challenges because the infection spreads to the aortic valve annulus, leading to an annular abscess that necessitates reconstruction. We present a straightforward miniskirt Bentall procedure using an annular pericardial patch to treat Bentall's endocarditis with an annular abscess. After removing the former composite graft and debriding the aortic root and annulus, we created a miniskirt-composite graft using a mechanical or bioprosthetic valve, a straight or Valsalva graft, and an annular bovine pericardial patch with a valve prosthetic sizer.

Article Synopsis
  • - A 61-year-old man with end-stage renal disease and a tunneled dialysis catheter presented for hypoxia after a recent hospital discharge for a neck abscess.
  • - Initial ultrasound upon admission showed a pericardial effusion without significant tamponade, but new hypotension developed during his stay, indicating worsening condition.
  • - Urgent pericardiocentesis was performed, draining a large volume of fluid, and interventional radiology suspected an endovascular injury as the cause of the hemopericardium related to catheter placement.
