In this manuscript, we comment on the article by Liu published in the recent issue of the journal. Hyperuricemia (HUA) has become the second most common metabolic disease after type 2 diabetes mellitus and is the most important risk factor for gout. This discussion focuses on the targets and clinical application value of traditional Chinese medicine (TCM) extracts in the treatment of HUA and gout, emphasizing the role of gut microbiota.

The recent comment on our previously published article questioned the novelty and computational efficiency of our work. Here we respond by restating the novelty and scientific value of our work as well as showing why the specific alternative methods stated in the comment are unlikely to outperform the methods we compare for metasurface applications involving high refractive index particles near high refractive index substrates.

I would like to thank Professor Ekirch for his reflections on 'Have we lost sleep?', which contain several points that I have already responded to within the paper following his peer review of my original submission to in 2023 (Professor Ekirch having voluntarily identified himself as a reviewer in a normally double-blind process). I acknowledge that the focus of my paper was on Ekirch's original work from 2001; if I did not engage as he would have wished with his subsequent publications, this was simply because I do not perceive the same substantial developments in his thinking and research on the subject that he does. Indeed, the present critique by Ekirch amounts essentially to more of the same: a long list of references and quotes but little detailed discussion of any individual source.

Objectives: According to French recommendations, only the caryotype is carried out as a first line in candidates for gamete donation. The prescription of additional genetic tests for variants responsible for serious monogenic diseases is only recommended in the case of call points. However, cystic fibrosis remains the most common genetic disease with serious consequences in childhood.
