Leptin, like estrogen, is one of the endo/paracrine factors, which are synthesized in and secreted from mature adipocytes. The roles of the mammary fat pad and mammary adipocytes in the initiation of lactation are not clear. In this study, we showed that combination of prolactin, leptin and estrogen elevated the expression of the milk protein beta-lactoglobulin. We also showed that after prolactin stimulate the secretion of leptin from the mammary fat, leptin upregulated the expression of estrogen receptor alpha in the mammary epithelial cells. Also, prolactin affected aromatase mRNA expression in the bovine mammary fat and we demonstrated that leptin and prolactin can affect cholesterol secretion from explants in culture to the medium. Therefore, we suggest that prolactin initiates estrogen expression (as represented by aromatase mRNA) in the mammary fat pad, whereas leptin stimulates estrogen receptor alpha expression in the mammary epithelial cells. We hypothesize that leptin and estrogen, secreted from the mammary fat regulate lactation after stimulation of prolactin.

Intermediate filaments (IFs) play a major role in determining and maintaining cell shape and anchoring intracellular organelles in place, in the tissues and organs of several species, starting from the early stages of development. This study was aimed at the immunohistochemical investigation of the presence, cellular localization and temporal distribution of the intermediate filaments keratin 8 (CK8), keratin 18 (CK18), keratin 19 (CK19), vimentin, desmin and laminin, all of which contribute to the formation of the cytoskeleton in the rat mammary gland during pregnancy, lactation and involution. On days 7, 14 and 21 of pregnancy (pregnancy period), on day 7 post-delivery (lactation period) and on day 7 post-weaning (involution period), under ketamine hydrochloride (Ketalar-Pfizer) (90 mg/kg) anesthesia, two mammary glands were fully excised from the abdominal region.

Reduction of very huge breast-gigantic macromastia-is a challenge to breast surgeons in choosing the right procedure to obtain an optimal outcome. The feasibility of a superomedial pedicle (SMP) with some modifications proves to be a good option to achieve a viable nipple areolar complex (NAC) with good size and shape after good resection above 800 g. Out of the 35 patients with 70 breast reductions, 15 can be considered gigantic macromastia with reductions above 800 g.
