Next generation bioelectronic medicine: making the case for non-invasive closed-loop autonomic neuromodulation.

Bioelectron Med

January 2025

SecondWave Systems Incorporated, Head Quarters, Minneapolis-Saint Paul, MN, 55104, USA.

The field of bioelectronic medicine has advanced rapidly from rudimentary electrical therapies to cutting-edge closed-loop systems that integrate real-time physiological monitoring with adaptive neuromodulation. Early innovations, such as cardiac pacemakers and deep brain stimulation, paved the way for these sophisticated technologies. This review traces the historical and technological progression of bioelectronic medicine, culminating in the emerging potential of closed-loop devices for multiple disorders of the brain and body.

Decision support guided fluid challenges and stroke volume response during high-risk surgery: a post hoc analysis of a randomized controlled trial.

J Clin Monit Comput

January 2025

Department of Anaesthesiology and Intensive Care, Bicetre hospital, Assistance Publique Hôpitaux de Paris (AP-HP), Le Kremlin Bicetre, France.

Intravenous fluid is administered during high-risk surgery to optimize stroke volume (SV). To assess ongoing need for fluids, the hemodynamic response to a fluid bolus is evaluated using a fluid challenge technique. The Acumen Assisted Fluid Management (AFM) system is a decision support tool designed to ease the application of fluid challenges and thus improve fluid administration during high-risk surgery.

To develop reliable, valid, and efficient measures of obsessive-compulsive disorder (OCD) severity, comorbid depression severity, and total electrical energy delivered (TEED) by deep brain stimulation (DBS), we trained and compared random forests regression models in a clinical trial of participants receiving DBS for refractory OCD. Six participants were recorded during open-ended interviews at pre- and post-surgery baselines and then at 3-month intervals following DBS activation. Ground-truth severity was assessed by clinical interview and self-report.

Closed-Loop Systems and Real-Time Neurofeedback in Mindfulness Meditation Research.

Department of Neurology, University of California San Francisco, San Francisco, California; Neuroscape, University of California San Francisco, San Francisco, California; Weill Institute for Neurosciences, University of California San Francisco, San Francisco, California. Electronic address:

Mindfulness meditation has numerous purported benefits for psychological well-being; however, problems such as adherence to mindfulness tasks, quality of mindfulness sessions, or dosage of mindfulness interventions may hinder individuals from accessing the purported benefits of mindfulness. Methodologies including closed-loop systems and real-time neurofeedback may provide tools to help bolster success in mindfulness task performance, titrate the exposure to mindfulness interventions, or improve engagement with mindfulness sessions. In this review, we explore the use of closed-loop systems and real-time neurofeedback to influence, augment, or promote mindfulness interventions.

Closed-loop oxygen usage during invasive mechanical ventilation of pediatric patients (CLOUDIMPP): a randomized controlled cross-over study.

Front Med (Lausanne)

September 2024

Department of Paediatric Intensive Care Unit, Dr Behcet Uz Children's Disease and Surgery Training and Research Hospital, Health Sciences University, Izmir, Türkiye.

Background: The aim of this study is the evaluation of a closed-loop oxygen control system in pediatric patients undergoing invasive mechanical ventilation (IMV).

Methods: Cross-over, multicenter, randomized, single-blind clinical trial. Patients between the ages of 1 month and 18 years who were undergoing IMV therapy for acute hypoxemic respiratory failure (AHRF) were assigned at random to either begin with a 2-hour period of closed-loop oxygen control or manual oxygen titrations.
