We report here a rare case of a concurrent occurrence of abscesses caused by in the prostate, seminal vesicles, and epididymis. A 71-year-old male presented to our hospital with urinary retention, and an indwelling urethral catheter was inserted. He remained afebrile until a revisit one month later when he developed a fever and left scrotal swelling.

Hypervirulent (hvKp) has attracted increasing attention in recent years. Diabetes and serotype K1 or K2 are risk factors for invasive liver abscess syndrome including liver abscesses and the metastatic complications such as bacteremia, meningitis, endophthalmitis, and necrotizing fasciitis. Simultaneous infections of the liver, lungs, prostate, brain, and eyes are exceedingly rare.

Case Presentation: The patient was an 80-year-old man who had undergone Bacillus Calmette-Guérin immunotherapy for non-muscle-invasive bladder cancer. Two months later, he developed an irregular pelvic mass surrounding the prostate and rectum with no fever.

Chronic bacterial prostatitis is generally difficult to treat. It may involve multiple courses of antibiotics and may be difficult to eradicate with high rates of recurrence. We present the case of a 33-year-old male patient with a previous history of renal tract calculi, stent insertions, and perinephric abscess with percutaneous drainage, which then led to a right nephrectomy.
