Peripheral arterial disease.

Br J Hosp Med (Lond)

Imperial Vascular Unit, Imperal College London, Charing Cross Hospital, London.

Published: October 2009

Peripheral arterial disease is commonly caused by atherosclerosis, and symptoms depend on the location and size of the affected artery, metabolic demands on the tissue, and the presence or absence of a collateral circulation. This article reviews the current evidence for the diagnosis and management of peripheral arterial disease.

Background And Aims: Peripheral arterial disease (PAD) is a cardiovascular disease affecting approximately 6 % of the population. The condition is associated with claudication, non-healing ischemic ulceration, and a systemic pro-inflammatory and pro-oxidative state. Recently, nutrition therapy is exploring immunonutrition as a novel dietary strategy to reduce the progression of atherosclerosis.

Characteristics, management and outcomes of central versus peripheral pulmonary embolism: a retrospective cohort study.

College of Medicine, King Saud Bin Abdulaziz University for Health Sciences, King Abdullah International Medical Research Center, and Intensive Care Department, King Abdulaziz Medical City, Ministry of National Guard - Health Affairs, Riyadh, Saudi Arabia.

Background: The location of thrombus in acute pulmonary embolism (PE) is a debatable prognostic factor. We compared the characteristics and outcomes of hospitalized patients with central versus peripheral PE.

Methods: This retrospective study evaluated patients with acute PE diagnosed by CT pulmonary angiography who were hospitalized between 01/01/2016 and 31/12/2022.

Peripheral arterial disease (PAD) is a common complication associated with diabetes, which can lead to foot ischemia. The condition is often accompanied by infection and necrosis, ultimately leading to diabetic foot ulcers and the risk of amputation. Brown adipose tissue (BAT) and its secreted cytokines play an essential role in the regulation of glucose homeostasis, the modulation of inflammatory responses, and vascular endothelial cell proliferation.

Background: Chronic Limb Threat Ischemia (CLTI) is a severe form of peripheral arterial disease characterized by various symptoms including nonhealing wounds, ulcers and gangrene ultimately leading to a possible amputation. Therefore, revascularization either through endovascular intervention (EVI) or surgical bypass (SB) is an important step in management. Literature review of various studies including Randomized clinical trials (RCTs), Meta-analysis and observational studies show varied results with some studies suggesting better outcomes with EVI while majority of the others favors superiority of SB.

The increasing prevalence of chronic limb-threatening ischemia and diabetes mellitus has led to a surge in lower extremity amputations, driven by the combination of peripheral arterial disease and extensive wounds. Although revascularization often addresses ischemia, severe wounds pose a significant risk of amputation. Flaps, which involve the transfer of vascularized tissue, can provide immediate closure of complex wounds, particularly those involving bone or tendon exposure, where skin grafts are insufficient.
