Total scapholunate dislocation with complete scaphoid extrusion: case report.

J Hand Surg Am

Division of Plastic, Reconstructive, Maxillofacial and Oral Surgery, Department of Surgery, Duke University Medical Center, Durham, NC 27710, USA.

Published: January 2010

A case of complete dislocation of the lunate and scaphoid resulting from a fall is reported. We are unaware of any previously reported case of simultaneous dislocation with the scaphoid completely extruded from the wrist at the time of injury. The patient was treated with a proximal row carpectomy.

Wrist parameters measured on direct radiography are essential in diagnosing scaphoid-related pathologies and treatment. Although computed tomography has been used to establish normative for scaphoid measurement, no research has focused on Indian norms. This study aimed to determine the distribution and normal limits of parameters measured in our population's posteroanterior (PA) and direct lateral radiographs of the wrist.

Degenerative wrist conditions, such as scapholunate advanced collapse and scaphoid nonunion advanced collapse, often require salvage procedures to reduce pain and improve function. For early stages of disease, both proximal row carpectomy and scaphoid excision four-corner arthrodesis are viable motion-preserving options. There remains controversy on which technique is superior.

Article Synopsis
  • - This study assessed the effectiveness of different surgical procedures (single-stage, two-stage, and salvage) for treating chronic perilunate dislocations in 15 patients from 2013 to 2019, comparing their outcomes with prior literature.
  • - Out of the patients, 13 underwent open reduction and internal fixation (ORIF), while 2 required salvage procedures; the approach transitioned from single-stage to more complex two-stage procedures using external fixators for better results.
  • - Post-operative evaluations showed mixed outcomes, with some complications like nonunion and avascular necrosis occurring, and the mean scapholunate angle reflecting varying degrees of wrist extension and flexion.

Three-corner arthrodesis for scapholunate and scaphoid nonunion advanced collapsed wrists: outcomes at long term follow-ups.

Injury

Service d'Orthopédie 1 et Traumatologie, Hôpital Roger Salengro, F-59000, Lille, France. Electronic address:

Article Synopsis
  • The study evaluated the outcomes of three-corner arthrodesis in 13 patients with wrist issues, focusing on clinical, functional, and radiological results over a mean follow-up of 6.7 years.
  • Results showed significant improvements in pain and functionality, with a high union rate of 92.9% for the wrists treated.
  • The findings suggest that three-corner arthrodesis is a viable long-term surgical option for patients with advanced wrist conditions, despite some experiencing mild signs of radio-lunate arthritis.

Radiopalmar ganglion cysts: prevalence, morphology, and clinical significance in wrist MRI.

Eur Radiol

December 2024

Department of Radiology, Balgrist University Hospital, Faculty of Medicine, University of Zurich, Zurich, Switzerland.

Objectives: To assess radiopalmar ganglion cysts' (RPG) prevalence, morphology, and clinical significance in consecutive patients.

Materials And Methods: In this retrospective single-center study, two radiologists assessed the presence of RPG and morphologic features on wrist MRI. Radiopalmar complaints and scapholunate ligament (SLL) tears were evaluated.
