We present an observation of a 42-year-old patient admitted in urgency for lower limbs weakness followed by paralysis. At physical examination the lower limbs were cold. Pulses of the lower limbs could not be palpated. Color Duplex ultrasonography revealing an embolus occluding both the infrarenal aorta and bilateral iliac arteries. The aortoiliac embolus was removed by balloon endarterectomy through the femoral arteries with evacuation of a transparent, gelatinous embolus followed by extensive fasciotomies. Two-dimensional echocardiography showed a mass in the left atrium. Pathological examination of the material embolic confirmed the diagnosis of the cardiac myxoma. The clinical course was marked by the development of a syndrome of revascularization; death occurred the fourth day of hospitalization. This case highlights the need for pre- and perioperative measures to be taken to overcome hyperkalemia and acute renal failure when revascularizing acute, massive, prolonged ischemia of the lower body.

Ischemic ulcers due to compromised microcirculation of the lower limbs cause painful ulcers that pose a challenge for the correct diagnosis and treatment. Livedoid vasculopathy, calciphylaxis, and Martorell's hypertensive ischemic ulcer are part of this group and present some similarities due to microvascular occlusive impairment. They are often misdiagnosed as inflammatory ulcers such as pyoderma gangrenosum and vasculitis.

Introduction: The higher prevalence of multiple chronic conditions and frailty among older adults may increase the physiologic demand required for wound healing after a major lower extremity amputation (LEA). After below knee amputations (BKA), patients generally have improved prosthetic fitting rates, postoperative ambulation, and quality of life compared to an above knee amputation (AKA). However, the benefit of a BKA must be weighed against the risk of wound complications.

Introduction: For lower extremity penetrating traumas (LEPT), the impact of race and insurance status, as a surrogate of socioeconomic status, is still not fully elucidated. This study aims to explore the relationship between these variables and the likelihood of receiving an amputation for LEPT to further identify disparities in trauma care.
