The Thyroid Swing: A Patient's Journey From Hypothyroidism to Hyperthyroidism.

Cureus

January 2024

Department of Internal Medicine, Endocrinology and Metabolism, Faculty of Medicine, King Abdulaziz University, Jeddah, SAU.

The flip from hyperthyroidism (HT), as observed in Graves' disease (GD) cases, to hypothyroidism associated with Hashimoto's disease (HD) has been widely recognized and documented. However, the converse is infrequently observed. The observed phenomenon is likely attributed to the major role played by thyroid-stimulating hormone receptor antibodies (TRAbs) in this alternating disease pattern and a substantial reduction in thyroid tissue mass that could be stimulated by these antibodies.

[The management of hypothyroidism during pregnancy.].

Recenti Prog Med

December 2023

IRCCS Istituto Auxologico Italiano, Ospedale San Luca, Milano.

During pregnancy, the need for thyroid hormone and its production increase: the presence of a concomitant thyroid disease, however, may prevent the maternal thyroid from meeting such increased demands for hormone production, which could lead to - or worsen - a state of hypothyroidism. Hypothyroidism in pregnancy could adversely affect the growth and development of the fetus as well as the course of the pregnancy itself. The present case report involves a 38-year-old woman with autoimmune hypothyroidism, who has been on levothyroxine replacement therapy since adolescence.

Summary: Rare patients who have both thyroid-stimulating hormone (TSH) receptor-stimulating and -blocking antibodies can develop 'pendulum swinging' thyroid dysfunction. A 9-year-old girl with Down syndrome was treated with carbimazole for Graves' disease. After 2 years of treatment, she became profoundly biochemically hypothyroid, and this persisted after carbimazole was discontinued.

Introduction: Sjogren's syndrome is an autoimmune disease that commonly involves exocrinopathy. Although studies have reported psychiatric manifestations resulting from Sjogren's syndrome, few studies have focused on such manifestations in pediatric patients. Herein, we present a case of an adolescent girl with depression and involuntary self-harm behaviors related to Sjogren's syndrome with central nervous system involvement.

Endokrynol Pol

January 2023

Department of Otorhinolaryngology and Laryngological Oncology, Faculty of Medical Sciences in Zabrze, Medical University of Silesia in Katowice, Zabrze, Poland.

Introduction: The aim of the study was to identify the prognostic factors and the relationship between vertigo and the results of objective assessment of the vestibular organ and the levels of thyroid status in patients with Hashimoto's thyroiditis.

Material And Methods: The study population consisted of 28 women with Hashimoto's thyroiditis and coexisting chronic vertigo. In all patients, audiological assessment of hearing (tonal audiometry and impedance audiometry), Dix-Hallpike manoeuvre, caloric test, and kinetic tests (rotary chair test and swing chair test) were evaluated.
