Obsessive-compulsive disorder (OCD) can be a severe and disabling condition with considerable variability in clinical presentation, course, and treatment response. Based upon demonstrated efficacy in clinical trials, selective serotonin reuptake inhibitors (SSRIs) and cognitive behavioral therapy (CBT) have become the treatments of choice for patients with OCD. By contrast, psychodynamic formulations and treatments are often considered irrelevant or contraindicated. In the present paper, the authors present five clinical cases of OCD where psychodynamic understanding and/or treatment was essential for optimizing outcome. The authors suggest that a careful psychosocial history and dynamic formulation can enrich understanding in patients with OCD and may sometimes point to alternative or supplementary treatments. Psychodynamic interventions may be considered for two subgroups of patients: those with late-onset OCD that coincides with interpersonal stressors, and those having borderline personality disorder, for whom specialized forms of treatment, such as dynamic deconstructive psychotherapy, may be indicated.

This case report discusses the treatment of a 42-year-old male with over a decade of treatment-resistant obsessive-compulsive disorder (OCD) and comorbid major depressive disorder (MDD). The patient underwent various pharmacological and psychotherapeutic treatments, including multiple antidepressants and cognitive-behavioral therapy (CBT), yet experienced only partial symptom relief. At baseline, the patient's depressive symptoms were severe, with a Hamilton Depression Rating Scale (HAM-D) score of 28, and his obsessive-compulsive symptoms were marked, with a Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) score of 34.

Introduction: PAdverse Childhood Experiences (ACEs) are associated with an increased risk of mental health issues in general, but their relationship with panic disorder (PD) and obsessive-compulsive disorder (OCD) has received less attention compared to borderline personality disorder (BPD). Dissociative experiences are significant predictors of increased symptoms, reduced treatment adherence, and poor prognosis in several psychiatric conditions, including PD, OCD, and BPD; still, their impact remains underexplored. This part of the study focuses on the overall efficiency of psychotherapeutic programs on treatment-resistant patients diagnosed with PD, OCD, and BPD (or combined), as well as the relationship between ACEs, dissociation rates, and treatment results.

Parent-led cognitive behavioral therapy (CBT) is an efficient, promising form of therapy that may be well suited for autistic youth with anxiety disorders. A recent clinical trial found that parent-led CBT - in which parents led their child through a guided CBT workbook with varying degrees of therapist support - was efficacious for reducing anxiety and associated functional impairment. While such findings demonstrate promise for future intervention development and dissemination efforts with this population, more work is needed to elucidate clinical factors that impact response to treatment as well as drop-out.
