Long-term results of 2-flap palatoplasty.

The 2-flap palatoplasty is a single-stage surgical procedure described more than 35 years ago. There are few reports in the literature describing longitudinal speech results for either a single surgeon or a single surgical technique. This paper presents speech outcomes spanning the 28 years that Dr. Kenneth E. Salyer has been performing the 2-flap palatoplasty.

Background: Various surgical methods have been developed for treating velopharyngeal insufficiency (VPI); however, the choice of surgical treatment is controversial. The pharyngeal flap, which is commonly used, has a high success rate but frequently leads to airway complications. Furlow double-opposing Z-plasty (DOZ) does not deform the velopharyngeal port; therefore, it is expected to show good speech outcomes while reducing airway complications if an appropriate indication is noted.

Objective: To describe the use of a barrier membrane in dogs for repair of congenital hard palate defects and closure of oronasal fistulae (ONF) remaining after previous cleft palate (CFP) repair.

Animals: 7 client-owned dogs.

Methods: The hard palate defect was closed with medially positioned flaps (Von Langenbeck technique) or pedicle flaps (2-flap palatoplasty) and a membrane composed of autologous auricular cartilage from the pinna or allogenous fascia lata underlying the mucoperiosteal flaps.
