The proportion of the population over the age of 65 is increasing. Reducing diseases that are common in this age group and that are associated with morbidity and mortality is a priority, if healthcare budgets are to be used cost-effectively. Syncope and falls are common in older people and are a major cause of healthcare expenditure. Clinical interventions are available, but the cost-effectiveness of those interventions is unclear.

Diabetic cardiac autonomic neuropathy (CAN) is caused by damage to the autonomic nerve fibers that innervate the heart and blood vessels, leading to abnormalities in heart rate control and vascular dynamics. CAN encompasses symptoms such as exercise intolerance, orthostatic hypotension, cardiac denervation syndrome, and nocturnal hypertension. Neurogenic orthostatic hypotension (nOH), resulting from severe diabetic CAN, can cause symptomatic orthostatic hypotension.

Objective: The association between alcohol consumption and increased injuries from falls is well established, but there is a lack of data on the prevalence of substance use by fall type. This study aims to describe the distribution of alcohol and drug involvement in injurious falls.
Methods: Using the 2019 National Emergency Medical Services (EMS) Information System data set, we identified 1 854 909 patients injured from falls requiring an EMS response and determined the fall location (eg, indoors or on street/sidewalk).

Methods: Recent publications on the following topics were reviewed; understanding of vasovagal syncope pathophysiology, tilt-testing methodology and interpretation, drug, ablation and pacemaker therapy.
Results And Conclusions: The vasovagal syncope field is very active in researching its pathophysiology, using it to gain better understanding of the process and applying this knowledge to therapy.

Reduced orthostatic tolerance is common following periods of bed rest that are associated with illness or surgery, putting individuals at higher risk for syncope and falls following hospitalization. Following menopause, mechanisms of female cardiovascular regulation change, which may be associated with sex-specific responses to orthostatic stress following bed rest. The purpose of our experiment was to investigate sex differences between healthy post-menopausal women and similar age men (age: 55-65 years) for their orthostatic tolerance and cerebrovascular responses to standing following bed rest.
