In this case report, we present a 35-year-old homosexual man with advanced HIV disease and disseminated tuberculosis (TB) who developed paradoxical TB immune reconstitution inflammatory syndrome (IRIS). Corticosteroids, vital in life-threatening IRIS, were initiated, resulting in symptom resolution but unmasking AIDS-associated Kaposi's sarcoma (KS). Diagnostic confirmation revealed disseminated KS, necessitating a comprehensive therapeutic strategy involving chemotherapy and thalidomide as a steroid-sparing agent.

Pancreatic tuberculosis (TB) is an uncommon extrapulmonary presentation of TB. Identification of coinfection with HIV may unmask not only disseminated TB but also immune reconstitution inflammatory syndrome (IRIS). We present the case of a 70-year-old Indian woman newly diagnosed with AIDS and pancreatic tuberculosis with miliary disseminated disease.

An initial diagnosis of synovial cyst was made, and she underwent multiple aspirations and symptomatic treatments without improvement. Repeat arthrocentesis showed a WBC of 56,000/μL with 61 % neutrophils and 34 % lymphocytes.
