The use of the double balloon endoscope for ERCP in patients with surgically altered anatomy has shown to be safe and feasible. This technique permits a variety of diagnostic and therapeutic possibilities. A case is presented of a 38-year old male, admitted with jaundice, 8 months after surgery for a malignant liver tumour with construction of a Roux-en-Y hepaticojejunostomy. A stenosis at the site of the biliodigestive anastomosis was diagnosed. Combining double balloon endoscopy with a percutaneous approach in a rendezvous procedure, it was possible to place a self-expandable biliary metal stent. This is the first report of a successful placement of a metallic stent using this technique.

: The objective of the current research is to assess the benefits that come with antegrade ureteral stenting coupled with imaging techniques in children and also provide a new classification based on the ureter's morphological elements. : Between 2011 and 2024, 107 antegrade stent placement procedures performed in 71 pediatric patients aged 0-12 years who could not undergo retrograde double-J stent placement were retrospectively analyzed. According to the morphologic structure of the ureter, four categories were classified as normal, straight/slightly angled, S-shaped, and spiral-shaped.

Purpose: The double-flap technique (DFT) is an anti-reflux reconstruction procedure performed after proximal gastrectomy (PG), but its complexity and high incidence of anastomotic stenosis are problematic. We conducted this study to demonstrate the efficacy of robot-assisted DFT, with refinements, to address these issues.

Methods: Surgical outcomes were compared between the following procedures modified over time at our institution: conventional open DFT (group O, n = 16); early robotic DFT (group RE, n = 19), which follows the conventional open PG approach; and late robotic DFT (group RL, n = 21), which incorporates refinements to the early robotic DFT technique by exploiting more of the robotic capabilities available.

While hematochezia is common in Crohn's disease (CD), severe gastrointestinal hemorrhage causing hemodynamic instability is rare. Strictures, another frequent complication, usually cause obstructive symptoms. We report the first case of hemorrhagic shock from ulcerated ileal strictures as the initial presentation of CD.
