Introduction: This report describes the use of a combination of a retrograde femoral nail and distal femur locking plate for the treatment of an open intra-articular distal femur fracture in a 99-year-old female. The purpose of this report is to highlight that nail-plate fixation constructs can be performed percutaneously and expeditiously even in extremely old patients; therefore, patient age should not be a limiting factor in choosing this construct to allow for immediate weight-bearing.

Case Report: The patient was a 99-year-old female who presented to the emergency room after a fall.

Article Synopsis
  • Intraarticular knee fractures are significant and need proper treatment for better recovery outcomes, with cleavage fractures being a rare type that involves limited areas of the distal femur without additional complications.
  • The article presents two case studies: one of a young adult treated successfully without surgery and another adolescent who required minimal-invasive surgical intervention.
  • Both patients achieved excellent healing and returned to full function, with follow-up periods of nine and eleven months showing positive results.

Context: Medial tibial stress syndrome (MTSS) is a common chronic injury of the lower-extremity in the physically active population. However, the risk factors for MTSS remain unclear.

Objective: This study identified the risk factors for MTSS and established the continuum model of lower-extremity alignments, range of motion, muscular strength, and gait kinematics affecting each other and MTSS development.
