Ruptured tendoachilles along with skin defect is a complex problem to reconstruct. Both things require a priority. Single stage reconstruction of ruptured tendoachilles tendon with skin cover using distally based superficial sural arterial flap allows us to perform both. This procedure gives excellent result, shortens the stay, thereby reducing the cost. This method is a simple solution to the complex problem like ruptured tendoachilles with skin defect. In this study, 6 patients with rupture of tendoachilles tendon due to penetrating injury, with skin defect are presented. The repair was done using aponeurotic part of tendoachilles tendon, taken from proximal part of tendoachilles in the midline measuring around 2 to 2.5 cm in width and 8 to 10 cm in length, with intact distal attachment. The tendon was turned upside down by 180 degrees and sutured to the distal stump of the tendoachilles tendon without tension. The skin defect was covered using distally based superficial sural artery flap in the same sitting. The follow-up period was 9 to 30 months. All patients showed good results. In one patient there was distal necrosis of 1.5 cm of the distally based superficial sural artery flap, which healed satisfactorily with conservative treatment. Single stage tendoachilles reconstruction can be used with good functional result and patient satisfaction.

Article Synopsis
  • Achilles tendon injuries are on the rise, often due to various causes, and treatment is challenging, particularly with significant tendon gaps; timely surgical intervention is crucial for recovery.
  • A study conducted at Mymensingh Medical College Hospital in Bangladesh aimed to evaluate the effectiveness of primary repair for open tendo-achilles injuries, with patients undergoing the Modified Kessler's surgical method within 12 hours of injury.
  • The results showed that 89.58% of patients could stand on tiptoes post-surgery, indicating excellent surgical outcomes, while a few experienced complications like severe infection affecting their mobility.

The Achilles tendon, the body's largest tendon, is often vulnerable to rupture, primarily as a result of sudden dorsiflexion of a plantar-flexed foot. This injury predominantly affects individuals in their youth and middle age. In this case series, we describe three middle-aged men with neglected insertional Achilles tendon ruptures, each presenting an average 10 cm defect.

Background: A pectoralis major (PM) transfer is a viable treatment option for patients with scapular winging due to long thoracic nerve (LTN) palsy not responding to nonsurgical management. However, the long-term outcomes remain unknown. Therefore, the purpose of this study was to evaluate the long-term outcome of shoulder function (ie, minimum follow-up of 10 years) and quality-of-life (QoL) of patients treated for scapular winging due to LTN palsy with a PM transfer.

Introduction: Retrocalcaneal pain in late adulthood and athletes has been attributed to insertional Achilles tendinosis (IAT). Another presentation of IAT is a degenerative Achilles tear, which can be partial or complete. Symptomatic patients with failed conservative management are treated by debridement and repair of the Achilles tendon.

Background: Non insertional Achilles tendinopathy [AT] is a degenerative condition that is prevalent in runners. 30% have no preceding history and many runners do not develop AT. Overuse, pronation, and compromised blood supply are hypothesised as causal.
