Objectives: The treatment of inflammatory bowel disease (IBD) often includes immunosuppressive medications, which may increase the risk of vaccine-preventable illnesses. We aimed to assess the impact of immunosuppression on immune responses to pneumococcal vaccination in patients with IBD.

Methods: The study design consists of a prospective controlled clinical trial. This study was carried out at a tertiary-care IBD clinic. The subjects for the study belonged to one of the following three groups: adult patients with IBD on combination TNF-blockers and immunomodulators (Group A), those without immunosuppressive therapy (Group B), and age-matched healthy controls (Group C). The treatment consisted of immunization with 23-valent pneumococcal polysaccharide vaccines (PSVs). The main outcome was immune response for five serotypes defined as a twofold or greater increase from pre-vaccination titers and > or =1 microg post-vaccination titer.

Results: Sixty-four subjects participated in the study: 20 in Group A, 25 in Group B, and 19 in Group C. Pre-vaccination titers were similar among the three groups. Vaccine responses were lower in Group A than in Group B (P< or =0.01 for four out of five antigens) and Group C (P<0.01 for all five antigens). Overall vaccine response was seen in 45, 80, and 85% of Groups A, B, and C (P=0.01), respectively.

Conclusions: Immune response to PSV-23 is impaired in Crohn's disease (CD) patients on combination immunosuppressive therapy but is normal among non-immunosuppressed patients. Given the unpredictable likelihood for immunosuppressive therapy, newly diagnosed patients with IBD should undergo vaccination before the initiation of immunosuppressive therapy.

Cultural differences in diagnosis and treatment perceptions: Turkish collectivistic representations of common mental disorders.

Department of Language and Communication, Centre for Language Studies, Radboud University, Nijmegen, The Netherlands.

Purpose: Attention-deficit/hyperactivity disorder (ADHD) is less diagnosed among Turkish children, and Turkish clients drop out more often from depression treatments than Dutch clients. This article proposes that cultural differences in collectivistic versus individualistic perceptions of getting an ADHD diagnosis and being treated for depression might explain these ethnic disparities, which have been explored in this study.

Methods: Nine focus group discussions with Turkish individuals and 18 interviews with primary mental health practitioners were conducted.

Equid alphaherpesvirus 4 (EqAHV4; , ; equine rhinopneumonitis virus) has seldom been associated with complications such as abortion and myeloencephalopathy, given the low tendency of this virus to induce viremia. We investigated the frequency of EqAHV4 viremia in horses with fever and respiratory signs. Case selection included all equids with EqAHV4 quantitative real-time PCR (qPCR)-positive nasal secretions (defined as EqAHV4 qPCR-positive cases) submitted to a diagnostic laboratory.

Introduction: A key factor influencing the duration of untreated psychosis is that young individuals typically do not seek help during their initial psychotic experiences. This online study aimed to explore the efficacy of preventive video interventions providing information on psychosis on the attitudes towards seeking mental health care among young adults from the general population.

Methods: Participants (N = 147) were randomised to one of the following online conditions: a short 3-min video of an empowered patient or of a psychiatrist describing different aspects of mental illness, a short control video or no video.
