For the success of clinical and translational science, a seamless interoperation is required between clinical and research information technology. Addressing this need, the Michigan Clinical Research Collaboratory (MCRC) was created. The MCRC employed a standards-driven Web Services architecture to create the U-M Honest Broker, which enabled sharing of clinical and research data among medical disciplines and separate institutions. Design objectives were to facilitate sharing of data, maintain a master patient index (MPI), deidentification of data, and routing data to preauthorized destination systems for use in clinical care, research, or both. This article describes the architecture and design of the U-M HB system and the successful demonstration project. Seventy percent of eligible patients were recruited for a prospective study examining the correlation between interventional cardiac catheterizations and depression. The U-M Honest Broker delivered on the promise of using structured clinical knowledge shared among providers to help clinical and translational research.

Background: Ketone bodies are metabolites produced during fasting or on a ketogenic diet that have pleiotropic effects on the inflammatory and metabolic aging pathways underpinning frailty in models. Ketone esters (KEs) are compounds that induce hyperketonemia without dietary changes and that may impact physical and cognitive function in young adults. The functional effects of KEs have not been studied in older adults.

Objectives: Ketone bodies are endogenous metabolites produced during fasting or a ketogenic diet that have pleiotropic effects on aging pathways. Ketone esters (KEs) are compounds that induce ketosis without dietary changes, but KEs have not been studied in an older adult population. The primary objective of this trial was to assess the tolerability and safety of KE ingestion in a cohort of older adults.

Reporting tumor genomic test results to SEER registries via linkages.

J Natl Cancer Inst Monogr

August 2024

Surveillance Research Program, Division of Cancer Control and Population Sciences, National Cancer Institute, Bethesda, MD, USA.

Article Synopsis
  • Precision medicine is increasingly important in cancer care, but tumor genomic data has been lacking in the National Cancer Institute's SEER Program, limiting research on molecular subtypes.
  • To improve this, the SEER Program has implemented a centralized process to link cancer cases in their registries with genomic test results from molecular labs, using specialized software and a trusted third party for data handling.
  • Recent linkages have included various OncotypeDX tests and results from other genomic classifiers, which facilitate the research community's access to valuable, de-identified data for cancer studies.

NCI SEER-Linked Virtual Tissue Repository Pilot.

J Natl Cancer Inst Monogr

August 2024

Surveillance Research Program, Division of Cancer Control and Population Sciences, National Cancer Institute, Rockville, MD, USA.

Background: The Surveillance, Epidemiology, and End Results (SEER) Program with the National Cancer Institute tested whether population-based cancer registries can serve as honest brokers to acquire tissue and data in the SEER-Linked Virtual Tissue Repository (VTR) Pilot.

Methods: We collected formalin-fixed, paraffin-embedded tissue and clinical data from patients with pancreatic ductal adenocarcinoma (PDAC) and breast cancer (BC) for two studies comparing cancer cases with highly unusual survival (≥5 years for PDAC and ≤30 months for BC) to pair-matched controls with usual survival (≤2 years for PDAC and ≥5 years for BC). Success was defined as the ability for registries to acquire tissue and data on cancer cases with highly unusual outcomes.

We live at a time of rapid and accelerating biodiversity loss and climate change that pose an existential risk to the environment, humanity, and social justice and stability. Governmental responses are seen by many citizens, including scientists, as inadequate, leading to an increase in civil protests and activism by those calling for urgent action to effect change. Here we consider the role(s) of scientists in responding to those challenges and engaging with policy given that when a scientist moves into political advocacy, reflecting their values and preferences, their objectivity and the value of scientific opinion may be seen as compromised.
