Objectives: Anatomical concepts derived by surgical management of intracranial aneurysms provided a pathway to the current integration and practice of aneurysm treatment. These important early developments, such as the surgical microscope in the late 1960s, the development of new surgical techniques and catheters and the evolution of imaging technology, have all provided a fundamental sequence to current clinical practice.

Material And Methods: The 1960s can be recognized as the dawn of the current era of aneurysm management. It was during this time period that the quest for novel methods of treatment that carried less risk and severity to the patients was being considered with comparable results.

Conclusion: The current management of intracranial aneurysms is a merge between derived knowledge of early generational concepts and current technology. A clear and conclusive understanding of these current achievements is, however, still in their infancy.

Mortality in Haemophilia Patients in India: A National Cohort Study.

Haemophilia

December 2024

Advanced Center for Oncology, Hematology and Rare Disorders (ACOHRD), K.J. Somaiya Super Speciality Hospital & Research Center, Somaiya Ayurvihar, Sion East, Mumbai, Maharashtra, India.

Introduction: Mortality and morbidity in persons with haemophilia (PWH) have decreased due to improved diagnosis and treatment along with comprehensive population outreach efforts, but the impact is not uniform in different countries.

Aim: The study aims to assess all-cause and intracranial haemorrhage (ICH)-specific mortality of PWH in India.

Methods: This is a retrospective, observational, multi-centric cohort study of 1020 haemophilia patients from three centres in India.

Prompt emergence from general anesthesia is crucial after neurosurgical procedures, such as craniotomies, to facilitate timely neurological evaluation for identification of intraoperative complications. Delayed emergence can be caused by residual anesthetics, metabolic imbalances, and intracranial pathology, for which an eye examination can provide early diagnostic clues. The sunset sign (or setting sun sign), characterized by a downward deviation of the eyes, can be an early indicator of raised intracranial pressure (ICP) or midbrain compression, as is commonly observed in states of hydrocephalus or periaqueductal or tectal plate dysfunction.

Erdheim-Chester Disease: A Case Report Exploring Multisystemic Involvement.

Cureus

November 2024

Neuroradiology, Unidade Local de Saúde Vila Nova de Gaia | Espinho, Vila Nova de Gaia, PRT.

Erdheim-Chester disease (ECD) is a rare, multisystemic, non-Langerhans cell histiocytic neoplasm predominantly affecting middle-aged males in their fifth to seventh decades of life. It often presents with nonspecific symptoms, leading to a delay in its diagnosis. We report a case of an 85-year-old male with multisystemic manifestations, including retroperitoneal, skeletal, vascular, cardiac, orbital, and central nervous system (CNS) involvement.

Background: Entropy quantifies the level of disorder within a system. Low entropy reflects increased rigidity of homeostatic feedback systems possibly reflecting failure of protective physiological mechanisms like cerebral autoregulation. In traumatic brain injury (TBI), low entropy of heart rate and intracranial pressure (ICP) predict unfavorable outcome.

The incidence of venous thromboembolism is low when risk stratification-based thromboprophylaxis is used after fast-track hip and knee arthroplasty.

BMC Musculoskelet Disord

December 2024

Department of Surgery, Hospital Nova of Central Finland, Wellbeing Services County of Central Finland, Jyväskylä, University of Eastern Finland, Kuopio, Finland.

Background: The optimal length of thromboprophylaxis after total hip or knee arthroplasty (THA and TKA) is unknown. Fast-track protocols have improved patient care and led to shorter immobilization and length of stay (LOS) after THA and TKA, thereby diminishing venous thromboembolism (VTE) risk. Here, we investigated risk stratification-based thromboprophylaxis after fast-track THA and TKA.
