Reoperation after infrainguinal arterial bypass may be difficult because of scar tissue from the previous bypass operation and usually involves removal of the old synthetic bypass graft. The authors describe a novel method of retaining the old thrombosed, externally reinforced polytetrafluoroethylene graft and using it as a tunnel for redo bypass using a vein.

Importance: It remains unknown whether outcomes of the Placement of Aortic Transcatheter Valves 3 (PARTNER 3) and Evolut Low Risk trials are comparable with surgical outcomes in nontrial settings, considering the added risk of concomitant cardiac operations.

Objective: To compare 30-day mortality and stroke incidences of patients in the surgical aortic valve replacement (SAVR) arm of low-risk trials with those of similar patients in the US Society of Thoracic Surgeons Adult Cardiac Surgery Database (STS ACSD).

Design, Setting, And Participants: A cross-sectional sampling study was conducted of adults in the STS ACSD with severe aortic stenosis at low surgical risk for AVR who underwent SAVR during the years low-risk AVR trials (PARTNER 3 and Evolut Low Risk) were enrolling (calendar years 2016-2018).

Utilization of cangrelor following coronary artery stent placement as a bridge to cardiac surgery has been previously described in the literature. However, the use of cangrelor as bridge therapy to cardiac surgery for endovascular revascularization is lacking. We describe a case involving a 47-year-old female who developed a left lower extremity tibioperoneal trunk non-obstructing arterial dissection following extracorporeal membrane oxygenation decannulation, requiring repair with a Viabahn endoprosthesis.

Left ventricular (LV) free wall rupture is a rare and often fatal complication of an acute myocardial infarction. We report the case of an LV free wall rupture after the induction of general anesthesia in an elderly woman who presented for a coronary artery bypass graft (CABG) procedure in the setting of an inferior wall ST elevation myocardial infarction (STEMI) four days prior. This case emphasizes both the differential diagnosis for and the management of refractory hypotension.

Background: The role of body composition as a risk factor for adverse outcomesduring coronary artery bypass surgery (CABG) has been controversial. The goal of this study was to evaluate the effect of body weight on mortality in patients undergoing CABG.

Method: Using a large NIS database and ICD-10 coding for different bodyweight categories, we evaluated the effect of cachexia, overweight, obesity, and morbid obesity on in-hospital mortality after CABG.

Objective: This study aims to investigate the clinical application value of the central venous-arterial carbon dioxide partial pressure difference (Pv-aCO2) in postoperative cognitive dysfunction(POCD) in patients with acute aortic dissection.

Methods: A retrospective analysis was conducted on the general data of 236 patients. Blood gas samples were collected from the arterial and venous lines at various time points during the surgery, including before and after the initiation of cardiopulmonary bypass (CPB), immediately after CPB initiation, before and after deep hypothermic circulatory arrest, 30 min after rewarming, and 5 min before weaning from CPB.
