Aim: To raise compliance of hypertensive patients with treatment scheme using individual chronotherapy.

Material And Methods: Hypertensive patients were divided into two groups: the study group consisted of 225 patients, the comparison group--125 patients. The protocol comprised examination of the circadian blood pressure profile (CBPP), complex of BP indices, personal reaction to the disease (the psychodiagnostic test).

Results: The normal type of the circadian BP profile (dipper) was found only in 44.4% patients; 36, 120.2 and 9.3% patients were non-dippers, night-peakers and over-dippers, respectively. Only 51.6% patients had adaptive types of reaction to the disease. After half a year of individual antihypertensive treatment CBPP improved, adaptation to the disease was registered 1.5 times more often. 76% patients developed stable motivation to long-term treatment of hypertension.

Conclusion: For adequate and safe chronotherapy of essential hypertension it is necessary to know an individual circadian rhythm of BP. Motivation to long-term pharmacotherapy is the best in patients with adaptive reaction to the disease. Education of hypertensive patients must be conducted in conditions of psychological comfort. Positive changes in life style and motivation to treatment should be promoted.

Understanding the Importance of the Small Artery Media-Lumen Ratio: Past and Present.

Basic Clin Pharmacol Toxicol

February 2025

Department of Biomedicine, Aarhus University, Aarhus, Denmark.

The media-lumen diameter ratio of small arteries is increased in hypertension, diabetes and obesity. It is likely that both shear stress on the endothelial cells, transmural pressure and smooth muscle cell tone are important for the altered vascular structure. However, the precise interaction and importance of these factors are incompletely understood.

Background: There are insufficient studies to determine whether sodium-glucose cotransporter type 2 inhibitors (SGLT2i) will help reduce early diabetic cardiomyopathy, especially in patients without documented cardiovascular disease.

Methods: We performed a single center, prospective observation study. A total of 90 patients with type 2 diabetes patients without established heart failure or atherosclerotic cardiovascular disease were enrolled.

Background And Hypothesis: Gut dysbiosis characterized by an imbalance in pathobionts (Enterobacter, Escherichia and Salmonella) and symbionts (Bifidobacterium, Lactobacillus and Prevotella) can occur during chronic kidney disease (CKD) progression. We evaluated the associations between representative symbionts (Bifidobacterium and Lactobacillus) and pathobionts (Enterobacteriaceae) with kidney function in persons with autosomal dominant polycystic kidney disease (ADPKD).

Methods: In this cross-sectional study, 29 ADPKD patients were matched to 15 controls at a 2:1 ratio.

The role of right ventricular (RV) dysfunction in pulmonary hypertension (PH) has garnered increasing interest in terms of outcomes. This systematic review and meta-analysis evaluated the prognostic utility of three-dimensional echocardiography (3DE) derived right ventricular ejection fraction (RVEF) in PH. A systematic review and meta-analysis were performed using MEDLINE, Embase, and Scopus databases for publications reporting the hazard ratio (HR) of 3DE-derived RVEF in PH patients for the clinical end-points of composite outcome or all-cause mortality.

Soluble Fms-like tyrosine kinase-1 polymorphisms associated with severe-spectrum hypertensive disorders of pregnancy.

Arch Gynecol Obstet

January 2025

Department of Population and Public Health Sciences, Keck School of Medicine, University of Southern California, Los Angeles, CA, 90089, USA.

Background: sFLT-1 has been implicated in the pathogenesis of HDP. We aimed to examine the role of maternal and fetal polymorphisms in risk of HDP and severe-spectrum disease.

Methods: Cases of HDP (143) and controls (169) from mother-baby dyads were recruited at the Los Angeles County Women's and Children's Hospital (WCH).
