Article Abstract

Giant pituitary adenomas (more than 60 mm) are a most difficult group of tumors of the pituitary. Surgery is practically the only treatment, visual functions being most vulnerable. The study showed that the procedures for removal of tumors through transsphenoidal approach (improved vision or no negative changes was noted in 78.2% of the patients) or made in 2 steps (a good result was obtained in 64.7%) presented the least risk. The most complete removal of a supracellular tumor portion irrespective of a chosen approach yields a better result than non-radical operations despite its potentially high traumaticity of visual nerves and optic chiasms.

Background: Giant pituitary neuroendocrine tumor (GPitNET) are challenging tumors with low rates of gross total resection (GTR) and high morbidity. Previously reported machine-learning (ML) models for prediction of pituitary neuroendocrine tumor extent of resection (EOR) using preoperative imaging included a heterogenous dataset of functional and non-functional pituitary neuroendocrine tumors of various sizes leading to variability in results.

Objective: The aim of this pilot study is to construct a ML model based on the multi-dimensional geometry of tumor to accurately predict the EOR of non-functioning GPitNET.

However, reaching above the diaphragma sella, especially for posterosuperior and retrocavernous orientation, still poses some challenges. Background: Ulnar mammary syndrome (UMS) is an autosomal dominant disorder caused by heterozygous pathogenic variants in the T-box transcription factor 3 (TBX3) gene. The phenotype is classically characterized by upper limb defects and apocrine/mammary gland hypoplasia. Endocrine abnormalities include hypogonadotropic hypogonadism (HH), partial growth hormone deficiency and dysmorphic features, while ectopic pituitary gland and various congenital anomalies have also been described.

Surgical resection of non-functioning pituitary neuroendocrine tumors (NF-PitNET) is associated with new onset hormonal axis (HA) dysfunction, and factors predicting HA dysfunction are controversial, especially in large and giant NF-PitNET. Thus, we evaluated the postoperative hormonal function and assessed factors affecting HA dysfunction in patients with NF-PitNET. This prospective observational study involved 50 patients who underwent endoscopic surgical resection of NF-PitNET in the Department of Neurosurgery (April 2023-March 2024).
