A 45-year-old woman reported multiple episodes of reversible left eye pain and diplopia stretching over 12 years. Ophthalmic examinations had repeatedly disclosed a left sixth cranial nerve palsy. Postcontrast brain MRI performed 3 weeks after clinical onset of the most recent episode demonstrated enhancement of the cisternal segment of the left sixth cranial nerve. Five months later, when symptoms and signs had largely abated, postcontrast brain MRI was normal. The clinical diagnosis satisfies the criteria for "ophthalmoplegic migraine." Although reversible cisternal enhancement of the third cranial nerve has been often described in this condition, this is the first report of cisternal enhancement of the sixth cranial nerve.

Data were obtained through routine health care at the UMC Ljubljana, Slovenia from 2005-2022, using clinical pathways. The patients were classified into three groups: i) radicular pain of new onset (N = 332); or ii) involvement of cranial nerve(s) but without radicular pain (N = 997); or iii) erythema migrans (EM) skin lesion(s) in conjunction with symptoms suggestive of nervous system involvement but without either cranial nerve palsy or radicular pain (N = 240).

Background: In small cell lung cancer (SCLC), limiting the radiation dose in the hippocampus area during preventive cranial irradiation (PCI) can reduce nerve injury and cognitive decline. This study was done to compare changes in cognitive functions between hippocampal-protected (3D-H) and non-hippocampal-protected (3D) patients during PCI.
Materials And Methods: the study group included 113 patients with SCLC qualified to PCI divided in two subgroups: 3D-H (n = 74) and 3D (n = 39).
