Clarithromycin is a broad-spectrum macrolide antibacterial agent which is effective both in vitro and in vivo against the major pathogens responsible for respiratory tract infections. Clarithromycin's principal metabolite is 14-(R) hydroxyclarithromycin (14-OH-clarithromycin). The other metabolite, namely 14-(S) hydroxyclarithromycin is inactive. The purpose of this study was to show the hydroxylation of CLA at the 14 position to form the R and S epimers and to determine the metabolic ratio of 14ROHCLA/CLA and 14SOHCLA/CLA for understanding the metabolization. This study suggest that in healthy adults, the individual variations in therapeutic responses to clarithromycin can be assumed by taking the drug and its metabolites ratios. Clarithromycin and metabolites ratios increase during metabolization.

Background: Toxicological studies suggest neonicotinoids increase oxidative stress and inflammation, but few epidemiological studies have explored these effects.
Methods: National Health and Nutrition Examination Survey (NHANES) 2015-2016 data were used to estimate associations between neonicotinoid exposure and inflammatory markers, including the C-reactive protein-to-lymphocyte count ratio (CLR), monocyte-to-high-density lipoprotein ratio (MHR), monocyte-to-lymphocyte ratio (MLR), neutrophil-to-lymphocyte ratio (NLR), derived NLR (dNLR), lymphocyte-to-monocyte ratio, platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII) using linear and multinomial logistic regression models. Sex was evaluated as a potential modifier.

This study isolated a myosmine-degrading bacterial strain J-6 from tobacco-growing soil. The identification of this strain revealed it to be a new species within the genus Sphingopyxis. Analysis of the myosmine degradation products by HPLC, preparative HPLC, and UHPLC-MS/MS identified 8 metabolites, among which 3-pyridylacetic acid (3-PAA), 5-(3-pyridyl)tetrahydrofuranone-2 (PTHF), and 4-hydroxy-4-(3-pyridyl)butanoic acid (HPBA) were three novel metabolites that were not previously found in microbial degradation of tobacco alkaloids.
