Bowel perforation is a rare but life-threatening complication of bevacizumab, a new anticancer treatment. Patients with bowel perforation usually present with acute abdominal symptoms. Here a case history is presented to highlight a pancreatic cancer patient on bevacizumab chemotherapy who developed empyema as the first manifestation of gastric perforation. This unusual presentation warns physicians that bevacizumab-related bowel perforation can arise as a thoracic complication, without typical gastrointestinal manifestations, in an advanced cancer patient.

The duodenal diverticulum is a relatively frequent entity whose diagnosis has been increased over time with the development of new diagnostic and exploratory techniques. Periampullary diverticula (PAD) were classified as type 1, 2, or 3 according to the position of the major papilla from the endoscopic view: type 1, the major papilla was located inside of the diverticula; type 2, the major papilla was located at the edge of the diverticula; type 3, the major papilla was located outside of the diverticula. Complications of duodenal diverticula include ulceration, bleeding, perforation and inflammation with intestinal obstruction.

Eosinophilic gastroenteritis is an uncommon, chronic, immune-mediated condition characterized by eosinophilic infiltration that can affect any segment of the gastrointestinal tract. Clinical manifestations depend on the different layers of the intestinal wall affected, which also allows its classification into three subtypes (Klein classification) (1): mucosal, which presents with abdominal pain, diarrhea, or vomiting; muscular, with obstruction or perforation; and serosal, classically with ascites. Diagnosis requires the demonstration of tissue eosinophilia with compatible clinical manifestations, after excluding other causes of eosinophilia.
