A method of forming the rectum reservoir after low anterior resection of the rectum has been developed, consisting in low anterior resection with mesorectumectomy, forming a reservoir from the colon, forming a rectosigmoid passage. The operation was made in 37 patients aged from 47 to 69 years for rectum cancer with the tumor localized in the middle and low ampular part. Complications in the early postoperative period appeared in 16.2% of the patients. There were no lethal outcomes. Mean time of staying at hospital after operation was 11.4 +/- 2.1 days. In the period from 3 to 5 years after operation the indices of fecal continence in patients of the main group and group of comparison (patients operated without forming the reservoir) by Wexner score were close to normal, making 4.6 +/- 0.4 and 3.1 +/- 0.5 scores.

A 36-year-old female with a remote history of spontaneous pneumothorax treated with talc pleurodesis presented with a large anterior mediastinal synovial sarcoma. The mass developed within a residual asymptomatic mediastinal space during a 2-year period. The patient underwent complete surgical resection and adjuvant chemoradiotherapy, which provide the best chance for survival.

A 17-year-old man with no past medical history was referred to our department for heaviness, numbness, redness, and swelling of the left arm. Imaging studies showed that while the left arm was elevated, a compression of the subclavian elements occurred. The final diagnosis was a vascular thoracic outlet syndrome caused by hypoplasia of the anterior part of the first left rib.

Tetralogy of Fallot with absent pulmonary valve and a nonconfluent left pulmonary artery is an uncommon combination usually associated with respiratory distress due to tracheobronchial compression by the aneurysmally dilated right pulmonary artery. Although application of the conventional technique is not feasible for this subset of patients, our novel "spiral plication" technique, wherein both the anterior and posterior walls of the dilated right pulmonary artery are resected in a spindle shape and plicated as if it is squeezed spirally, provides effective release of the obstructed airway concomitant with reconstruction of the left pulmonary artery confluence with growth potential.

Cardiac tumors are a rare entity with typically unspecific symptoms. We present the case of a 43-year old female patient with epigastric pain and palpitations with a long history of diagnostics prior to discovery of an epicardial mass. The epicardial tumor with arterial vascularization from the left anterior descending artery was resected in a beating heart procedure and was found to be a lipomatous soft tissue tumor.

Cellular schwannoma is a schwannoma subtype with histopathologic features resembling those of malignant tumors. We report a case of a cellular schwannoma arising at the resection margin of an anterior mediastinal leiomyosarcoma. An 88-year-old woman who had undergone resection of an anterior mediastinal leiomyosarcoma 2 years previously developed a mediastinal tumor at the resection margin, raising suspicion of leiomyosarcoma recurrence.
