Background: Anterior column realignment (ACR), using a lateral lumbar or thoracic interbody fusion (LLIF) approach to release the anterior longitudinal ligament (ALL), is a powerful technique to increase segmental lordosis. We here report our experience with the use of expandible LLIF cages for ACR.

Methods: Retrospective, single-center observational cohort study including consecutive patients treated by LLIF using an expandible interbody implant.

Article Synopsis
  • Hemodynamic instability and inappropriate postoperative pain perception (IPPP) pose significant challenges in managing patients undergoing elective open lumbar aortic aneurysm repair under general anesthesia, with inadequate opioid use risking adverse events.
  • In a study involving 75 patients, three types of postoperative analgesia were tested: thoracic epidural anesthesia with ropivacaine, thoracic epidural with bupivacaine, and intravenous metamizole/tramadol, with the impact on pain levels and hemodynamic stability measured.
  • Results showed that nearly half of the patients still experienced IPPP despite preventive measures, and while no acute pain was reported in the ropivacaine group, this was associated with noticeable hemodynamic instability; however

Acromegaly and gigantism are rare diseases, usually caused by a growth hormone-secreting pituitary adenoma, recently renamed GH-secreting pituitary neuroendocrine tumor (GH-PitNET). The transsphenoidal approach is the mainstay of treatment, although a non-negligible number of patients require a multimodal approach with neo-adjuvant or adjuvant medical and radiation therapy. Understanding the clinical complexity of acromegaly and gigantism is essential to improve treatment safety and success.
