We present out technique for harvesting the gastroepiploic artery (GEA). We use a Universal Stabilizer Arm and an assistant attachment to push the liver against the diaphragm, giving en enough working space to harvest the graft. Between January and December 2007, 99 isolated coronary artery bypass grafting (CABG)s were performed, and in 36 (36.4%) patients the GEA was harvested using this technique. The mean operation time was 251.1 +/- 40.5 minutes and the mean number of distal anastomosis was 3.6 +/- 0.8. The early patency rate of the GEA graft was 95%. Combined use of a Universal Stabilizer Arm and an assistant attachment provide good exposure for harvesting the GEA.

In Japan, the vascular grafts employed in coronary artery bypass surgery comprised 69% internal thoracic artery, 35% great saphenous vein, 3% right gastroepiploic artery, and 3% radial artery. Graft harvesting for coronary artery bypass grafting is deemed the foundational skill to be mastered in cardiovascular surgery training and is frequently executed by novice surgeons. However, while redoing the anastomosis is relatively straightforward, repairing a damaged graft is exceedingly challenging.

Objective: Vascularized lymph node transfer (VLNT) has become an important surgical technique in the treatment of lymphedema. Considering the different available regions available for flap harvest, we aimed to analyze different donor sites for VLNT with respect to donor site morbidity, impact on limb volume, and patient-reported outcome measurements (PROMs).
Methods: A single-center prospective study of all patients undergoing VLNT at the Department of Plastic Surgery and Hand Surgery of the University Hospital Zurich between September 2016 and 2023 was conducted.

Background: Vascularized gastroepiploic lymph node transfer (VGLNT) is a well-accepted surgical treatment for restoring physiological function in chronic lymphedema. However, the inclusion of substantial lymph nodes (LNs) in the flap remains uncertain. This study aimed to identify the anatomical basis for reliable flap harvest for VGLNT.

published in 2023 made recommendations for the selection of bypass vessels. The left internal thoracic artery-anterior descending branch anastomosis represents the accepted gold standard. Radial artery could achieve a better long-term patency rate and a reduction in adverse cardiac events compared to the great saphenous vein.
