Blood transfusion: friend or foe.

AACN Adv Crit Care

Surgical Critical Care Services, Washington Hospital Center, 110 Irving St, NW, 4B-42, Washington, DC 20010, USA.

Published: July 2009

The effectiveness of transfusions is often compromised by adverse reactions. Common transfusion reactions (hemolytic transfusion reactions, transfusion-related acute lung injury, transfusion-associated circulatory overload, transfusion-related immunomodulation) are reviewed, including pathogenesis, clinical and laboratory manifestations, and treatment. In addition, artificial blood substitutes are discussed as a way to mitigate the risk of transfusion-related morbidity and mortality.

January 2025

Donation and Policy Studies, Canadian Blood Services, Ottawa, Ontario, Canada.

Background And Objectives: Despite screening procedures, a few blood donors confirm positive for transfusion-transmissible infections and are deferred. Effective notification of laboratory results is essential to ensure that donors are advised of confirmed results and to seek medical care. Here we report results from post-notification interviews of Canadian Blood Services donors.

Background: Alloantibodies for the Rh blood group are the most immunogenic antibodies found in the Chinese population, typically causing acute or delayed hemolytic transfusion reactions and fetal and neonatal hemolytic diseases. Autoantibodies are generally considered nonspecific, and approximately 50% of warm antibodies are secondary to a variety of diseases, especially hematologic tumors. In this case report, a rare autoantibody that mimics anti-C and anti-e specificity was identified.

Article Synopsis
  • This study investigates the balance between avoiding platelet wastage and ensuring transfusion efficacy, focusing on the effects of platelet compatibility and product modification on reaction rates.
  • A review of over 21,000 platelet transfusions revealed a 1.33% reaction rate, with major incompatibility and irradiated products associated with increased odds of adverse reactions.
  • The findings suggest that both platelet compatibility and product modification independently impact transfusion reactions, informing future policies for selecting platelet products for patients.
