Purpose: Failure mode and effects analysis (FMEA) is a widely used tool for prospectively evaluating safety and reliability. We report our experiences in applying FMEA in the setting of radiation oncology.
Methods And Materials: We performed an FMEA analysis for our external beam radiation therapy service, which consisted of the following tasks: (1) create a visual map of the process, (2) identify possible failure modes; assign risk probability numbers (RPN) to each failure mode based on tabulated scores for the severity, frequency of occurrence, and detectability, each on a scale of 1 to 10; and (3) identify improvements that are both feasible and effective. The RPN scores can span a range of 1 to 1000, with higher scores indicating the relative importance of a given failure mode.
Results: Our process map consisted of 269 different nodes. We identified 127 possible failure modes with RPN scores ranging from 2 to 160. Fifteen of the top-ranked failure modes were considered for process improvements, representing RPN scores of 75 and more. These specific improvement suggestions were incorporated into our practice with a review and implementation by each department team responsible for the process.
Conclusions: The FMEA technique provides a systematic method for finding vulnerabilities in a process before they result in an error. The FMEA framework can naturally incorporate further quantification and monitoring. A general-use system for incident and near miss reporting would be useful in this regard.

Background: Recent evidence suggests that unawareness in Alzheimer's disease (AD) continuum can be explained by a failure of the connections between brain regions involved in accessing and monitoring self and other information. It has been demonstrated that AD patients with anosognosia have reduced network connectivity in the default mode network (DMN); in addition, stronger connectivity of bilateral anterior cingulate cortex (ACC) was showed to be associated with anosognosia in prodromal AD suggesting a possible role of this region in mechanisms of "adaptation" to anosognosia early in the disease. Therefore, we hypothesized that anosognosia in AD could be associated with an imbalance between the activity of the DMN and the salience network (SN) detectable using resting state functional magnetic resonance imaging (fMRI).

Based on the elastic-plastic theory, the analytical formula of the second invariant J of deviatoric stress at any point around the circular roadway under the non-uniform stress field is derived. The distribution law of J of surrounding rock under the three-dimensional non-isobaric stress field is studied by theoretical analysis and numerical simulation. Combined with the butterfly failure theory of surrounding rock of roadway, the close relationship between the distribution pattern of J and the distribution pattern of plastic zone is found, and the failure mechanism of surrounding rock is revealed.

Purpose: The main objective of this study was to assess the impact of a composite quality improvement intervention on mode of birth in nullipara term singleton vertex (NTSVs).
Material And Methods: This was an ambidirectional study following the implementation of the intervention to reduce cesarean section rate in NSTV by comparing two birth cohorts, pre-composite quality improvement intervention cohort (January 2013-December 2015) and post-composite quality improvement intervention cohort (January 2018-December 2020).
Results: In the studied periods, there was a total of 7713 NTSV births.
